Recently, I had a run-in with one of Minnesota's famous potholes, and my car sustained some damage. Fortunately, I saw on the news that the city could cover the costs if one fills out
these forms, which I've attach below. I have the notice of claim form filled out and the receipts detailing the cost of the damage. The experience, though startling, has been educational.
