CLINICAL EXPERIENCES FOR STi3DEI�iTS IN Ni3RSING
The Universify of Minnesota School of Nursing, hereinafter referred to as "UM
School of Nursing," and the St. Paul Health Depariment, hereinafter referred to as
"The Heaith Deparhnent;' by means of this AGIZEEMENT, do hereby agree as
follows:
I. General
A. The UM School of Nursing and the Health Department will plan
cooperatively for student clinical experiences and for the appropriate
orientation for students and faculty.
B. The usual iength of the iahnTatnry expexience-for_undpr�raduate students will
be a University of Minnesota quarter; in the case of graduate students and
selected undergraduate students, it may extend over a longer period, as
determined and agreed to by the parties.
C. When applicable, approximaEely four weeks prior to the beginning of the
quarter, the UM School of Nursing will provide a tentative schedule
indicating number of siudents and faculty member(s) responsible. The Hsalth
Deparhnent will retain final right of refusal to accept any or all students for a
particular quarter or placement.
D. The UM Schooi of Nursing will assume responsibility for detexmining that
students have had an annual Mantoux test and/or chest x-ray, Hepatitis B
vaccination, and that students are immune to xubella and rubeola.
E. The UM S�hool of Nursing will identify a faculty member and the Health
Department Director or his/her designee will act as coordinators in taking the
main responsibility for liaison.
F. This Agreement and the conduct of the program will be xeviewed annually by
the parties concerned; interim conferences also wiil be held as needed in the
interest of harmonious planning when program changes are considered.

G. This Agreement shall be effective from January 1, 1997 through Decembex 31,
1997. The agreement will automatically be xenewed for periods of one year,
for up to four additional years. In the event either party wishes to pxopose a
major change in/or termination of the Agreement, adeguate notice shall be
given. For a major change, three (3) calendar months' notice in writing
would be deemed adequate; in the event of termination, one calendar year's
notice will be given.
H. The L7M School of Nursing is commifted to the policy that all persons shall
have equal access to its programs, facilities, and employment without regard
to race, color, creed, religion, national origin, sex, age, marital status,
disability, public assistance status, veteran status, or sexual oxientation.
I. T'he UM School of Nursing shall conduct all criminal background studies
required pursuant to Minn. Stat. 245A.04. The LTM School of Nursing
certifies that a copy is available for the Health Department upon their request.
If a copy is not available, the student shall authorize the Health Department
to conduct the required studies.
II. Policies Relatin� to Conduct of the Experience
A. The LJM School of Nursing will plan for all student learning experiences.
Case selection will be made from among clients/patients made available by
the Health uepartment siaif. Haurs for class an<i lavoraiory praciire, course
content, methods of teaching, and evaluation of students' progress in
meeting course objectives are the responsibility of the UM Schooi of Nursing
faculty.
B. The Health Department has ultimate responsibility for the quality of care
given to its patients.
C. The UM School of Nursing faculty member responsible for a group of
students, and the Health Depazhnent personnel share responsibility for
interpreting the student program to the Health Department staff and helping
UM School of Nursing faculty and students understand current Health
Departxnent policies and practices. Student orientation will be arranged at a
time convenient to the Health Department
D. The iJM School of Nursing faculty member arranges for students to provide
periodic reports to the Health Department supervisor and f or staff nurse of
tr«-care being provided :or ei�nts/patie�tts�y �e:�ts. The He�lrh
Department staff will keep faculty and students informed as they gain
information about clientslpatients for whom students are caring. This will
insure timely communication between faculty, students, and the Health
Department staff regarding client/patient status and care requirements.

E. The Health Department will provide satisfactory assigmnent for students.
UM School of Nursing faculty and the Health Department staff will confer on
the selecEion of clients/ patients.
F. The IJM School of Nursing faculty members will meet with the Healfih
DeparEment staff for an evaluation of the cluucal experience. Students'
evaluations of the experience w�ill be provided.
G. The Heaith Department will provide conference space for faculty and
students as needed.
III. Costs
A. The Health Department will provide without cost to the UM School of
Nursing or its students the use of available the Health Department clinical
resources and facilities; services rendered to clients by students will accrue to
the Health Department's total service record.
B. The UM School of Nursing shall require that siudents carry hospitalization
and medical insurance. T'he Health Department will not be responsible for
hospitalization or medical costs incurred by the student during his/her
affiliation in the Health Department.
C. All Eransportation costs will be borne directly or indirectly by the student,
including responsibility for automobile liability insurance.
D. The University of Minnesota certifies professional liability insurance
coverage for UM School of Nursing students and faculty while at the Health
Department for the clinical experiences related to their educational program.
This insurance coverage applies to the students only when they are engaged
in assignments within the course and scope of theix duties as clinical nursing
students. The University of Minnesota shall provide on request to the Health
Department a Certificate of Insurance evidencing professional liability
coverage in the minimum amount of $1,000,000 each claimJ$3,000,000 annual
aggregate.
E. The University of Minnesota agrees to defend, hold harmless and indemnify
the Health Department its officers, employees, agents and representatives
against all claims for loss or damage to property or injury or death to persons
arising irom ihe n�gligent wrongful acts ar o-tnissicros o-f t; e �T�iversity of
Minnesota, its officers, employees, agents, representatives or students during
the performance of its obligations under this Agreement.
The Health Department agrees to defend, hold harmless, and indexnnify the
University of Minnesota, its officers, empioyees, agents; representatives and

siudents against all clauns for loss or damage to property or injury or death ta
persons arising from the negligent or wrongful acts or omissions of the
Health Depaxtment, its officers, employees, agents or repxesentatives during
the performance of its obligations under this Agreement.
F. No student shall be deemed to be an agent or employee of the Health
Department.

The University of Minnesota School of Nursing desires to enter into an Agreement with Saint
Paul Public Health to provide clinical experiences for students in their nursing program.
This clinical expexience wi11 benefit both the students in their exposure to the field of
public health, as well as the relationship between the University of Minnesota School of
Nursing and Saint Paul Public Health.

CLINICAL EXPERIENCES FOR STi3DEI�iTS IN Ni3RSING
The Universify of Minnesota School of Nursing, hereinafter referred to as "UM
School of Nursing," and the St. Paul Health Depariment, hereinafter referred to as
"The Heaith Deparhnent;' by means of this AGIZEEMENT, do hereby agree as
follows:
I. General
A. The UM School of Nursing and the Health Department will plan
cooperatively for student clinical experiences and for the appropriate
orientation for students and faculty.
B. The usual iength of the iahnTatnry expexience-for_undpr�raduate students will
be a University of Minnesota quarter; in the case of graduate students and
selected undergraduate students, it may extend over a longer period, as
determined and agreed to by the parties.
C. When applicable, approximaEely four weeks prior to the beginning of the
quarter, the UM School of Nursing will provide a tentative schedule
indicating number of siudents and faculty member(s) responsible. The Hsalth
Deparhnent will retain final right of refusal to accept any or all students for a
particular quarter or placement.
D. The UM Schooi of Nursing will assume responsibility for detexmining that
students have had an annual Mantoux test and/or chest x-ray, Hepatitis B
vaccination, and that students are immune to xubella and rubeola.
E. The UM S�hool of Nursing will identify a faculty member and the Health
Department Director or his/her designee will act as coordinators in taking the
main responsibility for liaison.
F. This Agreement and the conduct of the program will be xeviewed annually by
the parties concerned; interim conferences also wiil be held as needed in the
interest of harmonious planning when program changes are considered.

G. This Agreement shall be effective from January 1, 1997 through Decembex 31,
1997. The agreement will automatically be xenewed for periods of one year,
for up to four additional years. In the event either party wishes to pxopose a
major change in/or termination of the Agreement, adeguate notice shall be
given. For a major change, three (3) calendar months' notice in writing
would be deemed adequate; in the event of termination, one calendar year's
notice will be given.
H. The L7M School of Nursing is commifted to the policy that all persons shall
have equal access to its programs, facilities, and employment without regard
to race, color, creed, religion, national origin, sex, age, marital status,
disability, public assistance status, veteran status, or sexual oxientation.
I. T'he UM School of Nursing shall conduct all criminal background studies
required pursuant to Minn. Stat. 245A.04. The LTM School of Nursing
certifies that a copy is available for the Health Department upon their request.
If a copy is not available, the student shall authorize the Health Department
to conduct the required studies.
II. Policies Relatin� to Conduct of the Experience
A. The LJM School of Nursing will plan for all student learning experiences.
Case selection will be made from among clients/patients made available by
the Health uepartment siaif. Haurs for class an<i lavoraiory praciire, course
content, methods of teaching, and evaluation of students' progress in
meeting course objectives are the responsibility of the UM Schooi of Nursing
faculty.
B. The Health Department has ultimate responsibility for the quality of care
given to its patients.
C. The UM School of Nursing faculty member responsible for a group of
students, and the Health Depazhnent personnel share responsibility for
interpreting the student program to the Health Department staff and helping
UM School of Nursing faculty and students understand current Health
Departxnent policies and practices. Student orientation will be arranged at a
time convenient to the Health Department
D. The iJM School of Nursing faculty member arranges for students to provide
periodic reports to the Health Department supervisor and f or staff nurse of
tr«-care being provided :or ei�nts/patie�tts�y �e:�ts. The He�lrh
Department staff will keep faculty and students informed as they gain
information about clientslpatients for whom students are caring. This will
insure timely communication between faculty, students, and the Health
Department staff regarding client/patient status and care requirements.

E. The Health Department will provide satisfactory assigmnent for students.
UM School of Nursing faculty and the Health Department staff will confer on
the selecEion of clients/ patients.
F. The IJM School of Nursing faculty members will meet with the Healfih
DeparEment staff for an evaluation of the cluucal experience. Students'
evaluations of the experience w�ill be provided.
G. The Heaith Department will provide conference space for faculty and
students as needed.
III. Costs
A. The Health Department will provide without cost to the UM School of
Nursing or its students the use of available the Health Department clinical
resources and facilities; services rendered to clients by students will accrue to
the Health Department's total service record.
B. The UM School of Nursing shall require that siudents carry hospitalization
and medical insurance. T'he Health Department will not be responsible for
hospitalization or medical costs incurred by the student during his/her
affiliation in the Health Department.
C. All Eransportation costs will be borne directly or indirectly by the student,
including responsibility for automobile liability insurance.
D. The University of Minnesota certifies professional liability insurance
coverage for UM School of Nursing students and faculty while at the Health
Department for the clinical experiences related to their educational program.
This insurance coverage applies to the students only when they are engaged
in assignments within the course and scope of theix duties as clinical nursing
students. The University of Minnesota shall provide on request to the Health
Department a Certificate of Insurance evidencing professional liability
coverage in the minimum amount of $1,000,000 each claimJ$3,000,000 annual
aggregate.
E. The University of Minnesota agrees to defend, hold harmless and indemnify
the Health Department its officers, employees, agents and representatives
against all claims for loss or damage to property or injury or death to persons
arising irom ihe n�gligent wrongful acts ar o-tnissicros o-f t; e �T�iversity of
Minnesota, its officers, employees, agents, representatives or students during
the performance of its obligations under this Agreement.
The Health Department agrees to defend, hold harmless, and indexnnify the
University of Minnesota, its officers, empioyees, agents; representatives and

siudents against all clauns for loss or damage to property or injury or death ta
persons arising from the negligent or wrongful acts or omissions of the
Health Depaxtment, its officers, employees, agents or repxesentatives during
the performance of its obligations under this Agreement.
F. No student shall be deemed to be an agent or employee of the Health
Department.
