To move forward with your projecrt a 50% down payment is due upon signing. Checks should be made payable to Cornerstone Concrete LLC and
sent to PO Box 428, Lake Elmo, MN 55042. The balance is due the day the concrete is CUT. We charge a late fee of 1.5% interest per month on
all past due invoices.
A 3% surcharge will be added to all credit card payments. Cancelation of this contract will result in a $1,000 cancelation fee. Any down payment,
less the $1,000 cancelation fee, credit card surcharges, and financing fees will be refunded.
Below is a summary of the customers responsibilities under the Terms and Conditions. Please check your email for the full Terms and Conditions.
1. Mark all sprinkler heads. IF your project includes an extension, please move all sprinkler heads AND lines out of the extension area.
Cornerstone is not liable for damage done to ANY underground infrastructure.
2. Provide access to an electrical outlet and garden hose.
3. Concrete trucks require 14' of overhead clearance, and our employees will need 2' of walking and working space on each side of the
driveway.
4. Note where the old slab goes under existing brick, stone, veneer, and siding. Cornerstone is not liable for damage done to buildings when
removing old slabs that were poured under the siding or into the foundation.
5. Plan to be home during the "Set Up" and forming day.
6. On projects that require multiple concrete trucks, there may be a slight variance in color.
7. Clean up any dirt and debris smaller than a quarter.
8 Adjust the yard and landscaping to the height of the new driveway.

The existing driveway will be removed and disposed of properly. We will
pour new concrete at 4" thick and will use a 4500# concrete mix design.
The entire slab will be fiber reinforced. The concrete will be cured with e5
internal cure and seal. This additive makes your driveway chemical and
maintenance free for the life of the concrete. 1-3 days after the pour we will
sawcut control joints. Individual sections shall not exceed 6' x 6'. Crack
preventive steps are crucial in placing concrete. With the expansion cut
guidelines that we have in place, it has made Cornerstone Concrete in a
league of its own in controlling cracks.

Recycled Base Being we cannot see the sub-soil before tear out we are unable to pre-
determine if and how much base will be required for your job. If we find that
the soil is below industry standard (2-4 inches), or if we need to set your
new driveway to the correct elevation, it may require additional base to be
brought in. Included in this estimate is 8 tons of base. Any additional base
will be priced at $50/ton. If your entire project needs to be rebased (above
the 8 tons of base) this will cost you an extra $500.00. Once the driveway
has been torn out, it is not uncommon to uncover garage foundation issues.
We will repair the garage foundation issues at a charge of $300/yard of
concrete plus $75/man hour.

MINNESOTA LAW, YOU HAVE THE RIGHT TO PAY PERSONS WHO SUPPLIED LABOR OR MATERIALS FOR THIS IMPROVEMENT DIRECTLY AND DEDUCT THIS
