municipality within 180 days after the alleged loss or injury is discovered a notice stating the time, place, and circumstances thereof, and the amount of compensation
or other relief demanded.”

Traveling in Southbound lane on Lexington Ave. Near the Como Pavillion entrance there is a grouping of potholes that have
formed. Making repeated contact with the potholes caused damage to the vehicle suspension components. I could not continue
traveling on the road. I had to call for a tow truck to remove my vehicle from the road.

The impact from the multiple potholes dislodge the driver side tie rod, and strut rod. I was forced to call a tow truck.
