CALL TO ORDER:
Joan Johnson called the Meeting to order. Joan stated that they will be switching the order of the Agenda
slightly; Dr. Keith Henry will give his presentation first and Zack Hansen has graciously deferred to be
second. Joan requested that introductions be made by Committee members and guests.
HIV/AIDS BY KEITH HENRY:
Rob Fulton introduced Keith Henry, M.D., who is the head of our Infectious Disease Component, our
Sexually Transmitted Disease Clinic, and our HIV/AIDS services. Dr. Henry is also well known not just
throughout our community but throughout the United States and the world. Dr. Henry presented an
overview of HIV/AIDS and the changing course of the infection in our area. He distributed graphs and
charts of Non-AIDS HIV Cases and AIDS Cases in Ramsey County. Highlights of Dr. Henry's presentation
were: AIDS is a biologic terrorist; every minute of the day 6 people die of AIDS and 11 people become
infected; in Africa babies are becoming infected with AIDS because of the lack of a $2.00 medication
mothers can receive during labor that would cut the transmission rate of HIV in half; twenty years ago AIDS
was a white gay disease and now it has moved into the African American community; Minneapolis is ground
zero with 50% of the cases of AIDS in Minnesota and Saint Paul with 15%; new transmission of HIV is 48%
in Minneapolis and 12% in Saint Paul; CDC has projected that two-thirds of the people infected with AIDS
in the United States/world will die and one-third of the people infected won't; over the last ten years there
has been a decline in new HIV/AIDS infections by route of exposure for gay men and an increase in the
heterosexual group which includes a lot of African immigrants; 25% of the people reported with new HIV
infections in Minnesota last year were born outside of the United States; there has been an incredible
reduction in mortality due to OIS and antiretroviral therapy, highly active therapy or triple therapy; too many
people are failing their initial drug therapy regiments due to the number of side affects from the drugs; there
has not been a single person who lived and has been cured of AIDS; with the current therapy it would take
60 - 115 years or never to eradicate the AIDS virus from a person; resistance to drugs is a consent problem
along with the cost of drugs which can run between $12,000 to $15,000 a year; there is a problem with
adherence to drugs over time; there is a lot of interest in Directly Observed Therapy (DOT) for certain
patient populations; the harm to patients from taking herbal/over the counter drugs along with their regular
drug regiment; there is a big push in new drug development to make it simpler (once a day/a couple of pills)
and side affects such as fat redistribution and cardiorespiratory disease; and debates are ongoing regarding the
risks and benefits of delayed drug therapy versus early drug therapy.
Discussion was held on the $10 billion it would cost to provide AIDS treatment to all people in Africa for
one year versus the $48 billion spent since September 11 for terrorist bombing, and how it would result in an
84% decrease in deaths or 2.4 million deaths per year saved or in a four-year treatment for all of Africa over
7 million lives would be saved, and the military budget alone for 2003 is again $48 billion; transmission of
drug-resistance AIDS virus in the San Francisco population and elsewhere; his meeting with three medical
schools in the Bangor area of India that do not get along with each other and how he will be presenting them
with a model from a new South African clinic they opened in September, and the next day he is meeting with
government officials and some large public companies to see if they can get government and public industry
to offer their employee's HIV therapy and have it also include family members; and is the typical health care

PRESENTATION ON ORGANIZED COLLECTION OF SOLID WASTE IN RAMSEY COUNTY:
Rob Fulton introduced Zack Hansen, Manager of Environmental Health for Ramsey County. Zack stated
that a number of things that Dr. Henry mentioned about taking the long view and alining our money, etc.
could also be applied to some of what we will be talking about regarding solid waste. Garbage/solid waste is
an environmental and a public health problem. Ramsey County generates about a million tons of garbage
every year; that would be line of garbage trucks from here to Detroit. Zack distributed Why are Ramsey
And Washington Counties Exploring Public Collection of Solid Waste, Ramsey County and Washington
County News - Why Should I Care About What Happens to My Trash? - Environmental Concerns - the
Exploration of Public collection In Ramsey and Washington Counties, and Trash Today. Highlights of
Zack's presentation were: a brief history of garbage dumps during the 60's and 70's in Ramsey County and
Minnesota and the estimated half billion dollar's residents of Minnesota will be paying during the next 30
years to take care of those garbage dumps; management of waste in Ramsey County; US Supreme Court
statement that garbage is a commodity and the commerce clans provision of the constitution; how the volume
of waste in the County grew 18% over the last five years but the population only grew 5%; last year only
55% of aluminum cans in Ramsey County were recycled, that means around 117 million aluminum cans were
thrown away and if just half of those cans would have been recycled the energy savings would have powered
8,000 homes for a year; if just half of the corrugated cardboard, office paper, newspaper, junk mail, etc. that
gets thrown away every year were recycled it would save about a million trees; a joint county workshop in
April to present to them the report and findings for public collection; framework for discussion on how
public collection in the communities will be based on what concerns they have heard from people so far and

what the community prefers; the most important outcome from the study was public engagement and getting
people more aware of this issue that we all produce a lot of garbage, it all has to go somewhere and we are
all responsible for it and how as a community we have to find a solution; and they have received about 3,000
trash inserts so far since it started last week.
Discussion was held on Minneapolis public collection system; the subsidy in Minneapolis for the north side
system and how does that compare to what we do now; at what level does the legal authority exists and does
the County have the legal authority to enforce a public collection system on the cities within the county; the
controversy a couple of years ago surrounding the closing of the recycling plant run by Super Cycle;
clarifying the perception that non profits who do not pay taxes do not get their recycling picked up; and
where Saint Paul is at with all of this.
Rob thanked Zack for coming in and giving a great presentation.

LEGISLATIVE UPDATE:
Rob reported that Jane Norbin who is head of our Health Planning and Policy, was in Denver with the state
talking to CDC about Bioterrorism money and the likelihood is that Ramsey County could be receiving
around $500,000 for Bioterrorism preparation and the likelihood that the dollar amount for next year from
the federal government could increase. Rob spoke about the Public Health Emergency Powers Act and how
as it proceeds through committees it is being stripped of a number of its elements, for example they have
removed the section regarding isolation, quarantine, restrictions on movements, or required medical
treatment. He also talked about the budget; $160 million in SCORE funds might be pulled back in the
biennium; and whether or not swimming pools in family day care facilities have to be regulated.
Discussion was held on whether or not they were going to make a reporting requirement for public health
people who have been treating illegal aliens on do they have to report them or not.
A motion was made to adjourn the meeting. The motion was seconded. The motion carried. The meeting

The first week of April is Public Health Week in the United States. The American Public Health
Association establishes this week to help spread the word of the good works of the public health
profession. This year,the topic is"Healthy People in Healthy Communities." We have over 300 staff
including support staff and professional staff who each day work to assure that we have healthy
people in our healthy community.
We should be pleased that we live in what is one of the healthiest states in the U.S. Despite that high
ranking each year, we still have pockets of disparity in health status.
Our department is challenged to look out for the health of all people living in our community and we
focus on those most in need incoming refugees, low-income families, pregnant teens—as well as
those behaviors that impact on health—tobacco use,violence, and unprotected sex. I am very proud
of our staff In a recent survey, 95% of them said they liked the work they do and the work was
satisfying to them. This reflects the commitment to the work and fulfillment we get from doing the
work.
The history of our agency dates back to 1854 when the first Board of Health was created in Saint
Paul. That Board of Health and the Health Director focused on the issues of that day—clean water,
disposal of waste, and reduction of infectious diseases. The first public health ordinance was adopted
in 1887 focused on reporting of diseases,quarantines,vaccinations and the control of privies,garbage
piles, stables, commercial waste, and nauseous odors that were seen as agents in the spread of
disease. In 1911, a group of citizens created the St. Paul Baby Welfare Association. The purpose
of the organization was to provide relief and charity for the worthy poor with particular reference to
infants. The first activity of the Association was to establish milk stations and baby clinics to help
reduce infant mortality. The Association grew to become the Family Nursing Service, which was
incorporated into the County in 1973. Also, in 1920, the County Board hired their first nurse to
discover and report cases of TB and other communicable diseases. The County continued to provide
school nursing services to areas of suburban Ramsey for many years. In 1997, Saint Paul Public

Health and Ramsey County Department of Public Health joined together to create our current
Department. As you can see, we have not moved very far from our roots some 150 years ago.
Jane Norbin, Section Manager for Health Policy and Planning is spending part of her time working
with the MN Department of Health on the planning for bioterrorism in local public health agencies.
The Federal government has sent funds to the State and a large portion of those funds will come to
our department. Jane is assisting MDH in this process. We expect that we will receive up to
$500,000 or more over the next two to three years for preparation for a bioterrorism event.
We continue to follow the"bouncing ball"of the State budget deliberations. I hope that by our April
3 meeting, the ball will have stopped bouncing and we can see the impact on our budgets. At the
moment, the County is still expecting to have some impact in the 2002 budget year that will have to
be met by reductions in expenditures by each department. At the moment, we have a soft freeze on
filling vacancies that may remain in place for the balance of the year. We have not seen any
immediate impact of these vacancies in our service delivery.
