OUNCIL RESOLUTION—GENERAL FORM

RESOLVED, that out of the Worklmerit s Compensation Account
of the General Fund, Marie E. Rose, widow of Walter E. Rose,
employe of the Department of Public Safety, Bureau of Fire,
who sustained a fatal injury in the course of his employment
on January 14, 1957, be paid the sum of $38.77 per week,
payable every four weeks, commencing January 15, 1957, and
continuing during dependency until the total sum of $17,500.00
shall have been paid, together with the sum of $550.00 on
account of funeral expenses incurred, the first four week
installment of $155.08 being due and payable February 11, 1957.
