COUNQL RESOLUTION — GENERAL FORM

COMMISSIONER DATE
RESOLVED, that Thomas D. Moore be paid the sum of
$1,575.00 out of the Workmen's Compensation Account of the
General Fund in full settlement of his claim against the City
of Saint Paul for compensation resulting from his injury during
the last week of April, 1957, while in the employ of the Department
of Public Works, whereby he suffered a temporary total disability
until February 14, 1958, and 10 per cent permanent partial dis-
ability to his back; said sum of $1,575.00 to be paid at the
rate of $45.00 per week at intervals of four weeks, commencing
on February 17,1958.
