Effective June 1, 1958, an employee, once having attained eligibility for City pay coverage, shall each month thereafter during the terms of the City group contracts, continue to be eligible for such City pay cover- age so long as said employee, if on monthly payroll, is carried for at least one normal working day (for his type of position) or, if on a semi- monthly payroll, so long as said employee is carried on the regular City payroll for the first half of the month for at least one normal working day (for his type of position). An employee paid on a semi- monthly basis, who fails to appear on the first semi- monthly payroll of any month, or a monthly employee who fails to appear on a monthly payroll for any month shall be ineligible for City pay coverage for such month, and they shall be re- quired to make direct payments to Minnesota Mutual, the administrative agency, in order to maintain continuous coverage. 2. New employees of the City, except those in the ineligible classifications, employed on and after June 1, 1958, who devote at least half of their workin hours to City business (a work week to consist .of 40 hours after having been employed by the City for one month, shall be eligible to insure with respect to themselves and their dependents without evidence of insurability at their own expense, through payroll deductions, provided they make written ap lication and authorize payroll deductions within thirty (303 days after becoming eligible. Employees making application more than thirty (30) days after they become eligible may insure by furnishing satisfactory evidence of insurability, at their own expense, until they become eligible for - City -pay coverage. Effective upon passage and publication of this resolution, when employees are compelled to lose time from City employment because of service on jury duty, such time spent on jury service shall be considered as working time for the City for the purpose of determining eligibility for City group coverage. 4. During the term of the City group insurance policies, employees of the City eligible for coverage under the City-pay portion of the group program who are disabled and off regular City payrolls as a result of personal injury arising out of and in the course of their employment with the City shall be continued under City -pay coverage during the period they are receiving compensation payments from the City for temporary partial disability Nor temporary total disability and not otherwise. This provision shall be operative upon passage and publication of this resolution. BE IT FURTHER RESOLVED, effective June 1, 1958, that persons presently employed in the mechanical trades posi- tions, hereinafter specified, who are being paid under a program wherein their wages or salaries have been increased to provide an allowance for health and welfare benefits, shall be excluded from the City group program with respect to City contribution, but shall be eligible for coverage under the group program, including coverage of dependents, without evidence of insurability, provided they make applica- tion for coverage and authorize payroll deduction within thirty (30) days of the date of becoming eligible therefor, namely, on June, 1, 1958, on an employee- pay -all basis through payroll deduction: Boilermaker Electrician Electrician Foreman Electrician General Foreman General Blacksmith Glazier Lather Lighting Foreman Lighting Repairman Plasterer 0711 al to City Clerk CITY OF ST. Alarm Lineman Sheet Metal Worker St eamf itt er Street Lamp Worker Welder Blacksmith BE IT FURTHER RESOLVED, That employees insured for coverages under the City group program for which payroll deduction is authorized, who have insufficient earnings from which to make authorized payroll deductions, may pay premiums directly to the Minnesota Mutual Life Insurance Company, the administrative agency, in order to maintain continuous coverage. Such direct payments may be made for a period not exceeding six consecutive months, and thereafter conversion provisions must be exercised to maintain continuous coverage. BE_IT FINALLY RESOLVED, That the City Clerk is directed to transmit to the organizations carrying the City's group coverages copies of this resolution. Said organizations are thereupon required to accept the terms contained in this resolution by letter directed to the City Clerk, executed by an officer of such aforesaid organizations.
