PRESENTED BY ember 27011961, Ste Pau120k COMMISSIONE F -arine Insurance Company l - - -- 'e; to the City of Saint P that it hi prepared as service under the C ^ov:•rz,,e provisions ,73Q r :v en said compa l li•.0 . , 4, 1, -1....�. h it ori3 ssr f'ti i' u+ k' 1;I C WHEREAS, since adoption of resolution "dg3f'juncil, Council File No. 205285 on December 27, 1961, St. 'Paul Fire and Marine Insurance Company has, by letter to the City of Saint Paul, indicated that it is prepared as an additional service under the major medical coverage provisions under Agreement between said company and the City of Saint Paul to modify its previous proposal so that from and after March 1, 1962, a broader major medical plan will be available to City employees by payment by employees therefor, through authorized payroll deduction. It is proposed by St.'Paul Fire and Marine Insurance Company that those employees who presently are covered under the major medical coverage provisions of the Agreement may continue to retain their present coverage or they may, if they desire, elect to be covered under the broader major medical plan whereby the co- insurance factor is changed from the present 75/25% to 80/20% and whereby the presently deductible amount under such plan is decreased from $200 to $100 and the maximum benefit for each accident or sickness is increased from $5,000 to $10,000, the premium for coverage under the broader plan being $1.20 per month for employee only coverage and $3.00 per month for employee and dependent's coverage; and _. WHEREAS, it is proposed by St. Paul Fire and Marine Insurance Company, however, that from and after March 1, 1962, employees desiring to avail themselves of major medical coverage pursuant to the Agreement between said company and the City of Saint Paul and who are not covered under the present major medical coverage provisions must, if they desire major medical coverage, come under the broader plan as that will be the only plan offered by the company for City employees covered for the first time on or after March 1, 1962, under the major medical coverage provisions of the aforesaid agreement; now, therefore, be it COUNCILMEN Yeas Nays DeCourey Holland Loss Mortinson Peterson Rosen Mr. President,- Vavoulis SM 6-61 Adopted by the- Cound 19 �t .,. *�� Approved 19 In Favor Mayor Against 'PUBLISHED D ORIGINAL. OFFICE OF THE CITY CLERK COUNCIL RESOLUTION - GENERAL FORM PRESENTED BY COMMISSIONER DATr 2 RESOLVED, That the Council of the City of Saint Paul hereby concurs in the proposal of St. Paul Fire and Marine Insurance Company asset forth in,its letter of January 8, 1962, directed to Mr. C. H. Stevenson, Secretary, Welfare & Insurance Committee, City of St. Paul, over the signature of. W. B. Hershe, Secretary, Health Insurance Department, to provide City employees under the Agreement between said Company and the City of Saint Paul the aforementioned major medical plans on the terms and conditions hereinabove set forth; be it FURTHER RESOLVED, That as herein indicated with respect to the major medical coverage plan offered by St. Paul Fire and Marine Insurance Company, resolution of the Council, Council File No. 205285 is hereby modified and superseded to the extent hereinabove indicated; be it FINALLY RESOLVED, That the City Clerk is hereby directed to send a copy of this resolution to St. Paul Fire and Marine Insurance Company.
