WHEREAS, Janet M. Blade, widow of G� on A D lade,
deceased employe of the City of Saint Paul, Department of
Public Safety, Bureau of Police, has petitioned the Industrial
Commission of Minnesota for dependent's benefits under the
Workmen's Compensation Law, contending that the death of her
husband on June 14, 1963 arO s_bag as a result of a heart injury
sustained in the course of his employment with the City of
Saint Paul, and the City has denied that his death was caused
by a job - connected injury; and
WHEREAS, the office of the Corporation Counsel on behalf
of the City and said Janet M. Blade through her counsel James
R. McClure have negotiated a Stipulation for Settlement of her
claim against the City which would be in full, final and complete
settlement of any claim she may have under the Workmen's Compensation
Law, the terms of such settlement being as follows: That the
City shall pay to said dependent widow the sum of $10,462.00
in one lump sum, as dependency compensation, without interest
deduction, less the sum of $2615.50 for attorney's fees, which
latter sum shall be paid directly to James R. McClure; further,
,that the City shall pay to said dependent widow the statutory
allowance for burial fee, $550; further, that the City shall
pay directly to James R. McClure the sum of $85.00 as
reimbursement for expenses incurred with Dr. Moses Barron
in connection Ath these proceedings, $64.96 directly to A -1
Ambulance Service, Monterey, California, and $30.00 directly
to said dependent widow as reimbursement for expenses of last
illne s s, , of which latter sum she agrees to reimburse Blue Cross
for $26.00; further, that the City shall pay the reasonable worth
and value, for services rendered byIr. Osman H. Hull, of Carmel,
California, for autopsy and report, and of Dr. Donald M. Scanlon,
of Carmel, California, for medical services in connection with
employe's last illness, upon presentation of itemized bills by
said medical creditors, now, therefore, be it
RESOLVED, that the office of the Corporation Counsel is
hereby authorized to enter into such Stipulation for Settlement
