3 WHEREAS, adverse action was taken against the Massage Practitioner license held by Julie R.
4 Stoltman (License ID#20050002586) for the City of Saint Paul, by Notice of Intent to Suspend License
5 dated September 26, 2006, alleging licensee failed to submit current certificates of General and
6 Professional liability insurance for the coverage period of July 5, 2006 through July 5, 2007, and proof of
7 affiliation from the Jewish Community Center; and
9 WIIEREAS, the licensee did respond to the Notice of Intent to Suspend License to contest the
10 allegation or submit the required insurance information and proof of affiliation; and

I Z WHEREAS, Notice of Intent to Suspend License stated that if the licensee failed to contest the
13 allegation or submit the required insurance information and proof of affiliation by October 6, 2006, that the
14 matter would be placed on the consenf agenda to impose the recommended penalty; now, therefore, be it
15
16 RESOLVED, that fhe Massage Practitioner license held by Julie R. Stoltman is hereby suspended.

NOTICE OF INTENT TO SUSPEND LICENSE

premises located at 1375 St. Pau1 Avenue in Saint Paul

Dear Ms. Stoltrnan:
The Office of License, Tnspections and Environxnental Protection has recoxnmended
suspension of the Massage Practitioner license held by 3ulie R. Stoltman dlb/a Julie R. Stoltman
far the premises located at 1375 St. Paul Avenue in Saint Paul. The basis for the
recommendation is as follows:
On August 25, 2006, you received a notice from the Office of License,
Inspections and Environmental Protection stating that your license renewal
was being placed on hold until they received: 1) current ceriificates of
General and Professional Liability insurance for the coverage period of
July 5, 2006 through July 5, 2007 and 2) proof of affiliation from the Jewish
Community Center.
You were given until September 8, 2006, to submit the information and as of
today's date it has not been received.
At tliis time, you have three options on how to proceed:
1. You will need to submit the insurance and proof of affiliation information
requested to the Office of License, Inspections and Environmental �rotection at 8
Fourth Street East, Suite 200, St. Pau1, Minnesota 55101-1002 no Iater than
Friday, October 6, 2006. Information should be directed to the attention of
Christine Rozek.

2. If you wish to have a public hearing before the Saint Paul City Council, you will
need to send me a letter with a statement admitting the facts and requesting a
public hearing. We will need fo receive your letter by Friday, October 6, 2006.
T'he matter will then be scheduled before the City Council for a public hearing to
detenniue whether to suspend your license. You will have an opportunity to
appeaz befare the Council and make a statement on your own behalf.
3. If you no longer wish to do business in the City of Saint Paul, you may send a
written statement to that effect to the Office of License, Inspections and
Environmental Protection, 8 Fourth Street East, Suite 200, St. Paul, Miunesota
55101-1002 no later than Friday, October 6, 2006. Information should be
directed to the attention of Christine Rozek.
If you have not contacted me by that date, I will assume that you do not contest the
suspension of your license.
