IL RESOLUTION — GENERAL FORM
RESOLVED, that Joseph Kubiszewski be paid
the sum of $787.50 out of the Workmen's Compensation
Account of the General Fund in full settlement of his
claim against the City of Saint Paul for compensation
resulting from his injury of July 19, 1963 while in the
employ of the Department of Public Works, for an additional
5% permanent partial disability to the back >said employee
having been previoulsy paid for a 5% disability and it
appearing that said disability has now increased to a 10%
disability of the back.
