3 WI�REAS, adverse acrion was taken against the Sidewalk Contractor license held by William
4 Anderson, d/b/a Anderson Masonry & Concrete, Ina (License ID#19990002379) for the Ciry of Saint
5 Paul, by Notice of Intent to Suspend License dated August 10, 2006, alleging licensee failed to submit a
6 current copy of their Auto Liability Insurance far the coverage period of Apri125, 2006 through Apri125,
7 2007; and
9 WHEREAS, the licensee did respond to the Norice of Intent to Suspend License to contest the
10 allegation or submit the required insurance informafion; and
11
12 WHEREAS, Notice of Intent to Suspend License stated that if the licensee failed to contest the
13 allegation by August 21, 2006, that the matter would be placed on the consent agenda to impose the
14 recommended penalty; now, therefore, be it
15
16 RESOLVED, that the Sidewalk Contractor license held by William Anderson, d/b/a Anderson
17 Masonry & Concrete, Inc. is hereby suspended.

NOTICE OF INTENT TO SUSPEND LICENSE

RE: Sidewalk Contractar license held by William Anderson d/b/a Anderson Masonry &
Concrete, Inc. for the Ciry of Saint Paul.
License ID #: 19990002379
Dear Sir/Madam:
The Office of License, Inspections and Environmental Protection has recomxnended
suspension of the Sidewalk Contractor license held by William Anderson d/b/a Anderson
Masonry & Concrete, Inc. for the City_of Saint Pau1. The basis for the recommendation is as
follows:
On June 30, 2006, you were sent a notice from the Office of License
Inspections and Environmental Protection stating that your Sidewalk
Contractor license renewal was being placed on hold until you submitted a
current copy of your Auto Liability Insurance for the coverage period of
Apri125, 2006 through April 25, 2007 with the City of Saint Paul named as
addifional insured.
You were given until July 14, 2006 to submit this information. As of today's
date that information has not been received.

At this time, you have three options on how to proceed:

If you wish to continue to do business in the City of Saint Paul, you need to submit the
insurance information requested to the Office of License, Inspections and Environmental
Protection at 8 Fourth Street East, Suite 200, St. Paul, Miuuesota 55101-1002 no later
than Monday, August 21, 2006. This information should be directed to Christine Rozek.
2. If you wish to have a public hearing before the Saint Paul City Council, you will need to
send me a letter with a statement admitting the facts and requesting a public hearing. We
will need to receive your letter by Monday, August 21, 2006. The matter will then be
scheduled before the City Council for a public hearing to deteruiine whether to suspend
your license. You will have an opporhxnity to appear before the Council and make a
statement on your own behalf:
If you no longer wish to do business in the City of Saint Paul, you may send a written
statement to that effect to the Office of License, Inspections and Environmental
Protection, 8 Fourth Street East, Suite 200, St. Paul, Minnesota 55101-1002 no later than
Monday, August 21, 2006. Information should be directed to the attention of Christine
Rozek.
If you have not contacted me by that date, I will assume that you do not contest the
suspension of your license.
