PAUL OFFICE OF THE CITY CLERK UNCIL RESOLUTION — GENERAL FORM COUNCIL NO 46 WHEREAS, the Health and Welfare and Insurance Committee of the City of Saint Paul, at its meeting on September 9, 1964, adopted by unanimous vote of all members present a motion whereby said Committee recommends to the Council that the Cityts group insurance program with the present carriers be renewed through the calendar year 1965, Now, Therefore, be it RESOLVED, that the health and welfare program for employees of the City of Saint Paul as exemplified by contracts between the City of Saint Paul and The Minnesota Mutual Life Insurance Company, Minnesota Hospital Service Association, and St. Paul Fire and Marine :; nsurance Company, shall be continued in force for the period of one year from January 1, 1965, subject to all of the terms and conditions as provided in the contracts heretofore entered into between the aforementioned health and welfare program carriers and the City, except that the agreement between the City and the Minnesota Hospital Service Association shall be subject to modification and increase in premiums to $8.05 for employee coverage and $20.15 for dependents= coverage as set forth in the notice to the City from said carrier, said notification being received on July 1, 1.9647 as provided under the terms of its agreement with the City; be it FURTHER RESOLVED, that the Council of the City of Saint Paul, relative to the coverage provided by the Minnesota Hospital Service Association, directs the proper City officers, on behalf of eligible employees, to pay as the City -pay portion of such coverage $8.05 for employee coverage and $5.68 for dependentsr coverage; be it FURTHER RESOLVED that as to all City -pay coverages heretofore or herein authorized, the proper City officers are authorized to levy and collect a tax for the purpose of providing the necessary funds for payment of premiums or charges for such City -pay coverages, and such sums so levied and appropriated, aslprovided in M.S.A.
