3 WHEREAS, adverse action was nutiated against the Massage Pracfitioner license application
4 submitted by Raini Olson (License ID #20030003023) for the City of Saint Paul, by Notice of Intent to
5 Deny License Application dated April i l, 2006, alleging licensee failed to submit proof of successful
6 completion of the written and practical exams from the City of Saint Paul authorized exaininer; and
7
8 WHEREAS, the licensee did respond to the Notice of Intent to Deny License Application to contest
9 the allegation; and

11 WHEREAS, Notice of Intent to Deny License Application stated t1�at if the licensee failed to
12 contest the allegation by Apri124, 2006, that the matter would be placed on the consent agenda to impose
13 the recommended penalty; now, therefore, be it
14
15 RESOLVED, that the Massage Practitioner license application submitted by Raini Olson is hereby
16 denied.

Dear Ms. Olson:
The Office of License Inspections and Environmental Protection has recommended deniai

Paul. The basis for the recouunendafion is as follows:
On September 19, 2003, you were sent a letter from the Office of License,
Inspection and Environmental Protection stating that your license application was
incomplete and that you needed to provide proof of successful complefion of the
written and prachcal exams from the City of Saint Paul authorized egaminer.
On August 31, 2005, the inspector checked the records again and found that there
was no change in the writEen egam scores from November 4, 2003 and November 10,
2003. Also there were no practical exam aitempt results.
As a result of this information, it appears that you were unable to retake your exams
and receive a passing score.
At this time you hade three options on how to proceed:
� 1. If you wish to do business in the City of Saint Paul, you will need to submit the pzoof of
successful complefion of your written and practical exams to the Office of License,
Inspections and Environmental Profection at 8 Fourth Street East, Suite 200, St. Paul,
Mnlnesota 55101-1002 no later than Monday, April 24, 2006. Information should be
directed to the attenrion of Christine Rozek.

2• If you wish to have a public hearing before the Saint Paul City Council, you will need to
send me a letter with a statement admitting the facts and requesting a public hearing. We
will need to receive your letter by Monday, Apri124, 2006. The matter will then be
scheduled before the City Council for a public heariug to detennine whether to deny your
license applicafion. You will have an opporiwuty to appear befare the Council and make
a statement on your own behalf.
3. You can withdraw your license application. Send a written statement to that effect to the
Office of License, Inspections and Environmental Protecfion, 8 Fourth Sireet East, Suite
200, St. Paul, Mitmesota 55101-1002 no later than Monday, Apri124, 2006. Information
should be clirected to the attention of Christine Rozek. Any request for a refund o£ the
license application fee must also be made in writing to the Office of LIEP.
If you have not contacted me by that date, I will assume that you do not contest the
denial of your license application.
