WHEREAS, adverse action was initiated against the Massage Practitioner license held by
Jennifer A. Frey, d/b/a Jennifer A. Frey (License ID#20030004424) for the premises located at
340 Cedar Street in Saint Paul, by Notice of Intent to Suspend License dated February 16, 2006,
alleging that licensee failed to submit required insurance information; and
WHEREAS, the licensee did not respond to the Notice of Intent to Suspend License to
contest the allegation or submit the required insurance information; and
WHEREAS, the Notice of Intent to Suspend License stated that if the licensee failed to
respond by February 27, 2006, that the matter would be placed on the consent agenda to impose
the recommended penalty; now therefore, be it
RESOLVED, that the Massage Practitioner license held by Jennifer A. Frey, d/b/a
Jennifer A. Frey is hereby suspended until such time as the required insurance information is
received and approved by the Office of LIEP.

Dear Ms. Frey:

suspension of the Massage Practifioner license held by 7ennifer A. Frey, d/b/a Jennifer A. Frey
for the premises iocated at 340 Cedaz Street in Saint Paul. The basis for the recommendarion is
as follows:
On January 25, 2006, you were sent a letter from the Office of License,
Inspections and Environmental Protectiou requesting a current certificate of
General and Professional Liability insurance for the coverage peniod of
Apri129, 2005 through April 29, 2006 with a 30 day notice of cancellafion
and naming the City of Saint Paul as an additional insured.
You were given untit February 8, 2006, to respond. As of today's date, that
information has not been received.
At this tima, you have three options on how to proceed:
1. If you wish to continue to do business in the City of Saint Paul, you need to submit the
requested information to the Office of License, Inspections and Environmental Protection
at 8 Fourkh Street East, Suite 200, St. Paul, M.innesota SS1Q1-1002 no later than Mottday,
Febr¢aiy 27, 2006. Inforxnafion shouId be directed to the attention of Christine Rozek.

If you wish to have a public hearing befare the Saint Paul Cify Council, you will need to
send me a letter with a statement admitting the facts and requestittg a public hearing. We
wzll need to receive your letter by Monday, February 27, 2006. Tfie matter wilt fJien be
scheduled bafare the City Council for a public hearing to determi.ne whethez to suspend
your license. You wiIl have an opportwuty to appear befare tbe Council and make a
statement on your own behalf.
If you no longer wish to do business in the City of Saint Paul, you may send a written
statement to that effect to the Office of License, Inspections and Environmental
Profecrion, 8 Fourth Street East, Suite 200, St. Paul, Minnesota 55101-1002 no later than
Monday, February 27, 2006. Information should be directed to the attention of
Chrisfine Rozek.
If you have not contacted me bq that date, I will assume that you do not contest tLe
suspension of your license.
