WHEREAS, adverse action was taken against the Massage Practitioner license held by 3anet L.
Price (License ID #2005 0 002 8 3 1) for the City of Saint Paul, by No6ce of Intent to Suspend License dated
October 25, 2006, alleging licensee did not submit the required General and Professional Liability
insurance information; and
WAEREAS, the licensee did not respond to the Notice of Intent to Suspend License to contest the
allegation or submit the required insurance information; and

WHEREAS, Notice of Intent to Suspend License stated that if the licensee failed to contest the
allegation or submit the required insurance information by November 6, 2006, that the matter would be
placed on the consent agenda to impose the recommended penalty; now, therefore, be it

The Office of License, Inspections and Environmental Protection has recommended
suspension of the Massage Pracfitioner license held by Janet L. Price for the City of Saint Paul.
The basis for the recommendation is as follows:
On October 2, 2006, you were sent a letter from the Office of License,
Inspections and Environmental Protection stating that the renewal of your
license was being placed on hold until you submitted a current cert'if'icafe of
General and Professional Liability insurance for the coverage period of
August 13, 2006 through August 12, 2007, with a 30 day notice of
cancellation and namiug the City of Saint Paul as an additional insured.
You were given until October 16, 2006, to submit this information. As of
today's date, fhat information has not been received.
At tivs time, you have three options on how to proceed:
You will need to submit the insurance information requested to the Office of
License, Inspections and Environmental Protection at 8 Fourth Street East, Suite
200, St. Paui, Minnesota 55101-1002 no later than Monday, November 6, 2006.

2_ If you wish to have a public hearing before the Saint Paul Ciry Council, you will
need to send me a letter with a statement admiiting the facfs and requesting a
public hearing. We will tteed to receive your letter by Monday, November 6,
2006. The matter will then be scheduled before the City Council for a public
hearing to detenuiue whether to suspend your license. You will have an
opporLunity to appeaz be£ore the Council and make a statement on your own
behaLf.
3. Tf you no longer wish to do business in the City of Saint Paul, you may send a
written statement to that effect to the Office of License, Inspections and
Environmental Protection, 8 Fourth Street East, Suite 200, St. Paul, YvIinnesota
55101-1002 no later than Monday, November 6, 2006. Information should be
directed to the attention of Christine Rozek.
If you have not contacted me by that date, I will assume that you do not contest the
suspension of your license.
