FILE OFFICE OF THE CITY CLERK CO NCIL RESOLUTION — GENERAL FORM PRESENTED BY COMMISSIONER DATE _ WHEREAS, the Council of the City of Saint Paul has heretofore provided for a comprehensive health and welfare and insurance program for City employees and employees of the Board of Education of the City of Saint Paul and for employees of the Board of Water Commissioners of the City of Saint Paul, all of whom are herein- after referred to, for convenience, as City employees; and WHEREAS, it presently appears desirable to consolidate in one document the rules and regulations heretofore adopted with reference to coverage of City employees under the aforesaid comprehensive health and welfare and insurance program, now, therefore, be it - RESOLVED, That effective as of December 31, 1966, the following rules and regulations shall apply with reference to coverage, in- cluding the City -pay coverage, of the officers and employees of the City under the City's comprehensive health and welfare and insurance program and contracts: 1. It is the determination of the Council of the City of Saint Paul that all officers and employees of the City of Saint Paul who presently are eligible for and covered under the City's health and welfare and insurance program shall continue to be eligible for the coverages to which they are presently entitled, subject to the terms, conditions and provisions of the afore- mentioned insurance contracts and further subject to the terms, conditions and provisions hereinafter set forth. Inclusion or exclusion of all new City employees, including provisional -- employees and excluding from coverage temporary and emergency employees and other employees as hereinafter set forth, shall be determined in accordance with the provisions of the specifica- tions controlling the contracts, which contracts became effective March 1, 1958, and as modified by agreements betweenthe City and its insurance carriers, which modifications are reflected by riders or indorsements to said contracts which have been executed by said insurance carriers, namely: Minnesota Hospital Service Association, Mercury Indemnity Company, and The Minnesota Mutual Life Insurance Company, and the City. fM COUNCILMEN _ Adopted by the Council 19— Yeas Nays Carlson , Dalglish Holland Meredith Peterson Tedesco Mr. President, Byrne In Favor Against Approved 19._ /4 ,r Mayor X22 Page 2. 231269 2. Excluded Classifications - Employees felling within the following classifications are to be excluded from the City group insurance program both with respect to coverage paid for by the City and with respect to coverage paid for by the employee: Substitute teachers Emergency employees Temporary employees Auditorium employees who are paid on a performance basis Parking lot attendants at Midway Stadium ,Library pages other than full -time Library pages Student Library Aide I Student Library Aide II .Recreation Leader I Life Guard _ .Water Safety Instructor Park Aide I Park Aide II 1 _Parking Lot_Attendant I Recreation Aide Substitute Library Technician Helper (Alarm and Lighting) Any employee who is participating as the insured in any other governmental health and welfare program for which contribution'is being paid by the employer Any employee whose services are furnished to the City or a department or board of the City, or to the Board of Education of the Cit of Saint Paul (Independent School District No. 625 under written contracts between-such employee and the City or any board or department thereof, or said Board of Education, and whose salary is paid in accordance with the provisions of such contract or agreement and whose name is not carried on the usual payrolls of the City, unless such employment contract expressly provides that the employee shall be included in the City and Board of Education group insurance program._ 3. New Employees - New employees who work not less than twenty hours.a week, except those in classifications specifically excluded from the City group insurance program by the provisions of this resolution) after having been employed by the City for one month, shall be eligible to insure with respect to themselves and their dependents without evidence of insurability at their own expense through payroll deductions, provided they make written application and authorize payroll deductions within thirty (30 days after becoming eligible. Employees making application more than thirty (30) days after they become eligible may insure by furnishing satisfactory evidence of,,insurability, at their own expense, until they become eligible for City -pay coverage. An employee upon becoming eligible for City -pay Page 3. 231269 coverage may insure with respect to his dependents at his own expense, through payroll deductions, without furnishing evidence of insurability (either as to himself or his dependents) provided he makes written - application and authorizes payroll deductions within thirty (30) days of the date of such eligibility; there- after, satisfactory evidence of insurability at his own expense may be required. 4. Eligibility - A (1) Any employee other than a provisional employee or other than an employee in a. classification specifically excluded from the City group insurance program by the provisions of this resolution who is paid on a bi- weekly basis, that is, paid every two weeks, in order to be eligible for City -pay coverage must appear on the payroll for an aggre- gate of 13 bi- weekly payroll periods and must have been on each uueh ayroll for at least five full working days (M hours); provided, that any time prior to separation from the City service (loss of reinstatement eligibility) shall not be'considered in determining the-13 aggregate payroll periods. (r2) Any provisional::emp.loyee other than a provisional employee in a classification specifically'excluded from the City group insurance program by the provisions of this resolution who is paid on a bi- weekly basis, that is, paid every two weeks, ,in order.to be eligible for City -pay coverage must appear on the payroll for 13 consecutive bi- weekly payroll periods and must have been on each such payroll for at least five full working days (40 hours) . (3) Upon the completion of the 13th bi- weekly payroll period under (1) or (2) above, an employee,.shall' become eligible-for City -pay coverage on the first day of the payroll period following the completion of the 13th period, but in no case earlier than-six months from the date of employ- ment, provided he is on the payroll at that time; otherwise, he shall be eligible for City - pay coverage on the day he returns to active work in his City employment status. B (1) Any employee other than an employee in a classi- _ fication specifically excluded from the-'Cit"O i t =J G 231269 Page 4. group insurance program by the provisions of this resolution who is paid on a monthly basis, that is, paid once a month, in order to be eligible for City -pay coverage, mast appear on the payroll for an aggregate of six monthly payroll periods and must have been on each such payroll for at least half time; provided, that any time prior to separation from the City service (loss of reinstatement eligibility) shall not-be considered in determining the six aggregate payroll periods. (2) upon completion of th period, such employee City -pay coverage on period following the period; provided he i otherwise he shall be on the day he returns employment status. e sixth monthly payroll shall become eligible for the first day of the payroll completion of the sixth s on the payroll at that time; eligible for City -pay coverage to active work in his City --When-employees-are compelled-to—lose time from City employment because of service on jury duty, such time spent on jury service shall be considered as working time for the City for the purpose of determining eligibility for City group coverage. 5. Continuing Coverage - A An employee, once having attained eligibility for City - pay coverage shall, each month thereafter, continue to be,,eligible for such City -pay coverage so long as said employee, if on a monthly payroll, is, carried for at tleast one (1) norma1fworking day (for his type of position) or, if on a bi- weekly payroll, so long as said employee is carried on the payroll for the pay - roll period occurring during the first portion of each month for at least one (1) normal working day (for his type of positions}'; the pay periods for each month of January through December from which such eligibility for City -pay coverage shall be determined shall be established for each year by the Council by separate resolution. Such employee paid bi- weekly who fails to appear on the designated bi- weekly payroll for any month as shown on the.table adopted by the Council, or an employee paid monthly who fails to appear on a payroll for any month, shall not be eligible for Cit y- paycoaVerage* for such month and shall be required to make direct payments to The Minnesota Mutual Life Insurance Company, the administrative agency,-in order to maintain continuous coverage:- �I Y`, r' i 7 1 s ttr t 231269 Page 5. B Such employee shall continue to be eligible for City -pay coverage so long as said employee works not less than twenty hours a week. C Such employee who has been granted sabbatical leave and who continues to be paid a part of his salary by the City during such leave, shall continue to be eligible for City -pay coverage. D Such employee who is disabled and off regular City payrolls as a result of personal injury arising out of and in the course of his employment with the City shall continue to be eligible for City -pay coverage during the period he is receiving compensation pay- ments from the City for temporary partial disability or temporary total disability and not otherwise. E Such employee, if a teacher, or an employee having regular civil service status, whose employment because of the period of the school year contemplates absence from the payrolls during the summer months, shall continue to be eligible for City -pay coverage during such absence. F Such employee insured for coverages under the City group program for which payroll deduction is authorized, who has insufficient earnings from which to make authorized payroll deductions, may pay premiums directly to The Minnesota Mutual Life Insurance Company, the administrative agency, in,order,,to maintain continuous coverage.
