Dear Sir/Madam:
The Office of License Inspections and Environmental Protection has recommended adverse action
against Ta�ucab Driver (Provisional) License application snbmitted by Amos W. Rogers dlbla Amos W. Rogers
for the City of Saint Paul. The basis for the recommendatiott is as follows:
On July 21, 2005 you were sent a letter by the Office of License, Inspections and
Environmental Proteciion requesting a copy of your drivin� record from the state of
Missouri. You were gjven nntil August 2, 2Q05 to submit the information. As of today's
date, the informarion has not been submitted.
The Office of LTEP will recommend denial of your license application until such time as the requested
documents are received.
At this time you have three options on how to proceed:
If you do not dispute the above facts but wish to do business in the City of Saint Paul, you may subznit
the requested documents to the Office of License, Inspections and Environmental Frotection at 8 Fourth
Sh East, Suite 200, St. Paul, Minnesota 55101-1Q02 no later than Monday, August 22, 2005.
Information shouid be directed to the attention of Ckuistine Rozek.

2. 7f you do not dispute the above facts, but wish to have a public hearing before the Saint Paul
City Council, you will need to send me a letter with a statement admitting the violafion and requesting a
pnblic hearing. We will need to receive your letter by Monday, Augnst 22, 2005. The matter wi11 then
be scheduled hefore the City Council for a public hearing to detern�iue what penalty, if any, to impose.
You will have an oppoztunity fo appeaz before the Council and make a statement on your own behalf:
3. You may withdraw your license application. Send a written statement to Ehat effect to the Office of
License, Inspections and Environmentai Protecfion, 8 Fourth Street East, Suite 200, St. Paul, Nfinnesota
55101-1002 no later than Moaday, Angust 22, 2005.. Information should be directed to the attention
of Ctuistine Rozek. Any request for a refund of the license appfication fee must also be made in writing
to the Office of LIEP. .
Please let me know in writing no later than Nionday, August 22, 2005, how you would like to
proceed. If I have not heard from you by that date, I wiil assume that you are not contesting the facts of
the violation. The matter will then be scheduled for the St. Paul City Council and placed on the Consent
Agenda during which time no discussion is allowed and your license application will be denied.
