License ID #:20040001015 Deat Sir/1Vladam: The Office of License, Inspections and Environmental Protection has recommended adverse action against the Solid Waste Hauler & Velucle License held by A Plus Pmfessional Grade, Inc., d/b/a A Plus Professional Grade Inc. for the City of Saint Paui. The basis for the recommendation is as follows: On June 21, 2005, you received a letter from the Office of License, Inspection and Environmental Protecfion requestin� the followin� informahon: 1) copy of current certificate of General Liability insurance for the covera¢e period of March 23, 2005 throngh March 23, 2006; 2) copy of a vehicle inspection report from Public Works Garage and 3) a completed Schedule of Changes form. You were �iven untit July 5, 2�05 to submit these documents. As of today's date these documents have not been received. The O�ce of LIEP will recommended a suspension of your license until such time as the requested documents have been received. All Information should be directed to the attention of Christine Rozek. 2. If you do not dispute the above facts, but wi sh to have a public hearing before the Saint Paul City Council, you will need to send me a letter with a statement admitting the violation and requesting a public hean�. We will need to receive your letter by Monday, August 1, 2005. The matter will then be scheduled before the City Council for a public hearing to determine what penalty, if any, to impose. You will have an opportunity to appeaz befare the Council and make a statement on your own behalf: 3. If you no longer wish to do business in the City of Saint Paul, you may send a written statement to that effect to the Office of License, Inspections and Environmental Protection, 8 Fourth Street East, Suite 20Q St. Paul, Minnesota 55101-1002 no later than Monday, August 1, 2005.
