Hagen for the premises located at 1672 Grand Avenue in Saint Paul License ID # 20050000981 Dear Sir/Madam: The Office of License Inspections and Environmental Protection has recommended adverse action against Massage Practitioner License application submitted by Harris J. Hagen, d/b/a Harris 3. Hagen for the premises located at 1672 Grand Avenue in Saint Paul. The basis for the recommendation is as follows: On March 29, 2005 you were sent a letter from the Office of License, Inspecrion and Environmental Protection iequesting the following information: 1) proof of snccessful completion of the Practical E%amination; 2)provide an insurance certificate showing covera¢e in General Liability and Professional Liability with the City of Samt Paul named as an additional insured and a 30-day norice of cancellation. You were �iven until Apri112, 2005 to submit the requested informarion. As of today's date, the information has not been received. Tke Office of LIEP will recommend denial of your license application until such time as the requested documents aze received. AA-ADA-EEO Employer � Iiarris J. If you do not dispute the above facts but wish to do business in the City of Saint Paul, you may submit the requested documents to the Office of License, Inspections and Environmental Protection at 8 Fourth Street East, Suite 200, St Paul, Minnesota 55101-1002 no later than Tnesday, Jnly 5, 2005. Informaiion should be directed to the attention of Christine Rozek. 2- If you do not dispute the above facts, but wish to ha�e a public hearing before the Saint Paul City Council, you will need to send me a letter with a statement admitting the violation and requesting a public hearing. We will need to receive your letter by Tuesday, July 5, 2005 . The matter will then be scheduled before the City Council for a public hearing to detemune what penalty, if any, to impose. You will have an opporiunity to appear before the Council and make a statement on your own behalf. You may withdraw your license application. Send a written statement to that effect to the Office of License, Inspections and Environmental Protection, 8 Fourth Street East, Suite 200, St. Paul, Minnesota 55101-1002 no later than Tuesday, July 5, 2005. Informafion should be directed to the attention of Christine Rozek. Any request for a refund of the license application fee must also be made in writing to the Office of LIEP. Please let me know in writing no later than Tuesday, July 5, 2005, how you would like to proceed. If I have not heard from you by that date, I will assume that you are not contesting the facts of the violation.
