RESQLVED, That pursuant to the Award of the Industrial. Commission,
dated May 31, 1966 , Clair Reitz be paid the swn of $9 ,000.00 out of� the
Workmen's Compensation Aceount of the General Fund, representing payment
of compensation for 52 weeks of temporary total disability and 1.48 weeks
for a 60 per cent pe�nanent partial disability to his right arm and
approximately 10 per cent permanent partial disability to his back, all
at the rate of $45.00 per week, for disability resulting from his injuries
of August 1.2 , 1947, May 10 , 1961, and June 11, 196q�, while employed by
the Department of Public Safety; that of said sum, $6 ,750.00 be paid
_ � directly to Clair Reitz and the sum of $2 ,250.00 to his attorney of record,
Gerald B. Forrette, as and for attorney�s fees herewith;
FURTHER RESOLVED, That from and after May 16 , 1966 , Clair Reitz be
�aid compensation benefits at the rate of $45.00 per week, subject to
° the p ovisions and limitations of the Minnesota Workmen's Compensation
� Act said ,payments to be made in the following raanner: To the employee
the sum of S33. 75 , and to his attorney of record,the swn .of $11.25 ; and
that the sum of $190.OU be paid directly to Dr. Meyer Z. Goldner in ful.1.
payment for his examination and testimony;'� that when said payments are
made,it shall constitute a full, final and complete settlement of any
and all claims that the employee may have for compensation resulting
from his injuries of,August 12 , 1947, May _10 , 1961, and June 11, 196�+.

the Department of Public Safety; that of said sum, $6 ,750.00 be paid
directly to Clair Reitz and the sum of $2 ,250.00 to his attorney of record,

the sum of $33. 75 , and to his attorney of record,the sum of $11.25 ; and
that the sum of $190.00 be paid directly to Dr.. Meyer Z. Goldner in full
payment for his examination and testimony; : that when said payments are
made,it shall constitute a full, final and complete settlement of any
and all claims that the employee may have for compensation resulting
from his injuries of Au�ust 12 , 1947 , May 10 , 1961, and June 11, 1964.
