WHEREAS, adverse acrion was initiated against the Massage Practitioner licence held by
Dominic J. Carchedi, d/b/a Dominic J. Carchedi (License ID#20040001384), for the premises
located at 656 Selby Avenue in Saint Paul by Notice of Intent to Suspend License dated November
23, 2005, alleging failure-to submit current insurance certificates and proof of affiliation; and
WHEREAS, licensee did not respond to the Notice of Intent to Suspend License to contest
the aliegation or submit the requested information; and
WHEREAS, the Notice of Intent to Suspend License stated that if the licensee failed to
respond or submit the requested information by December 5, 2005, that the matter would be placed
on the consent agenda to impose the recommended penalty; now, therefore, be it
RESOLVED, that the Massage Practitioner license held by Dominic J. Carchedi, d/b/a
Dominic J. Carchedi is hereby suspended.

Dear Mr. Carchedi:
The Office of License Inspections and Envixonmental Protection has recontmended the
suspension oPthe Massage Pracfitioner license held by Dominic J. Carchedi, d/b/a Doxninic J.
Carchedi for the premises located at 656 Selby Avenue in Saint Paul. The basis for the
recommendation is as follows:
On October 21, 2005, you reeeived a letter from the Office of License,
Inspections and Environmental Protection stating that your license renewal
had been placed on hold uniil the:following docnments are received: l) a
current certificate of General Liability and Professional Liability insurance
for the coverage period of Apri114, 2005 through Apri113, 2006 with a 30
day notice of cancellation arid the City bf Saint Paui named as additionai
insured; 2) written proof of affiliation from a licensed massage facility.
You were given until November 7, 2005, to submit these documents. As of
today's date, they have not be received.
At this time you have three options on how to proceed:
1. You can submit the necessary information to the Office of License, Inspections
and Environmental Protection at 8 Fourth Street East, Suite 2D0, St. Paul,
Minnesota 55101-1002 no ]ater than Monday, December 5, 2005. Information
shouid be directed to the attenfion of Christine Rozek.

2• If you wish to have a public hearing before the Saint Paul City Council, you will
need to send me a letter with a statement admitting the facts and requesting a
public hearing. We will need to receive your letter by Monday, December 5,
2005. The matter will then be scheduled before the City Council for a public
hearing to deteiiuine whether to suspend your license. You will have an
opporhxnity to appeaz before the Council and make a statement on your own
behalf:
3. If you no longer wish to do business in the City of Saint Pau1, you may send a
written statement to that efFect fo the Office of License, Inspections and
Environxnental Protection, 8 Fourth Street East, Suite 200, St. Paul, Minnesota
55101-1002 no later than Monday, December 5, 2005. Information should be
directed to the attention of Ctuistine Rozek.
Please let me know in writing no later than Monday, December 5, 2005, how you
would like to proceed. If I have not heard from you by that date, I will assume that you
are not contesting the suspension of your license.
