` RESOLVED, That pursuant to the Award of the Industrial Commission,
dated June 1� 1967, Frank X. Bennett be paid the sum. of $765.00 out of
the Workmen's Compensation Account of the General Fund, representing 17
weeks of compensation at the rate of $45.00 per week for compensation
accrued between Feb.ruary- 7 , 1967, and June 6 , 1967, for disability re-
sulting from his injury of November 27 , 1966 � while employed by the
Department of Public Safety, Bureau of Police; that of said sum, $5'7•3. 75
be paid� directly to Frank X. Bennett and the sum of $191.25 to his �
attorney, Robert E. Faricy? _in full satisfaction for all legal services
, rendered to June 6 , 196 7.
FURTHER RESOLVED, That from and after June 6 , 1967, Franlc X. Bennett
be paid compensation benefits at the rate of $�5.00 per week for the
duration o� his permanent and total disability, said payments to be made
in the following manner: To the employee, the sum of $33.75 per week
and to his attorney, the sum of $11.25 ; that the employer shall re-
imburse said employee the sum of $422.05 for medical expenses he has �
incurred, and pay directly to Drs. Briggs and Bel].omo the sum of $387-.'80 ;
that when said sums shall have been paid, it sha11 constitute full, final
and complete settlement of any and a11 employee's claims herein for
personal injury or occupational disease in the nature of hypertensive
heart disease or coronary athersclerosis or insufficiency and disability
therefrom-occurring on or about November 27, 1966.
