The basis for the recommendation is as follows: You were given an inspection notice by the Office of License, Inspections and Environmental Protection dated October 15, 2003 indicating that you needed to submit license fees for the current year by October 22, 2003 Additionally, the license information on file shows that the business at this address is Safari Hair Adventure but the name at the location is "Beautiful Spa". No licenses exist for you to operate under that name. The Office of LIEP will recommend that your license to operate in Saint Paul be suspended until such time as you pay your license fees and indicate whether you have changed any of the information that would affect your license. At this time you have three options on how to proceed: 1.
