Dear Ms. Lazson:
The Office of License Inspections and Environmental Protection (LIEP) has
recommended denial of the above-referenced license application. The basis for the
recommendation is as follows:
You were sent a letter by the Office of License, Inspections and
Environmental Protection on January 28, 2004, indicating that
LIEP had received notice from the bank that your check #3346
in the amount of $3,067.00 was dishonored due to nonsufficient
funds and that your license application was incomplete and
required additionai information. You were to provide
payment in the amount of $3,082.00 (including a$15.00
returned check fee) and all required information by February
2, 2004, but as of today's date nothing has been received, nor
have you indicated that you no longer wish to do business in
Saint Paul.
The recommendation is for the denial of your license application. If you do not dispute
the above facts but wish to proceed with your license application, you may submit the required
information to the Office of LIEP by Mazch 25, 2004. Please direct the information to Christine
Rozek at the Office of LIEP, 350 St. Peter Street, Suite 300, Saint Paul, MN 55102.

If you wish to dispute the facts, you can request a contested hearing, which will be
scheduled before an Administrative Law 7udge. At that hearing both you and the City will be
able to appeaz and present witnesses, evidence, and cross-examine the other's wihiesses. The St.
Paul City Council will ultimately decide the case.
If you wish to withdraw your license application you may do so. Any request for a refund
of the license application fee must be made in writing to the Office of LIEP.
Please let me lmow in writing no later than Thursday, Mazch 25, 2004, how you wish to
proceed. If you have not contacted me by Thursday, March 25, 2004, I will assume that you
are not contesting the facts stated above. I will then schedule this matter for the St Paul
City Councii and have it placed on the Consent Agenda during which no public discussion
is allowed and your license application will be denied.
