RESOLVED, that the Massage Center A License held by Julia Elliott, d/b/a Angelo's
Hair Care, for the premises located at 2028 St. Clair Avenue, in the City of Saint Paul, is hereby
suspended immediately for failure to pay delinquent license and late fees. Said suspension shall
remain in effect until such time as all license fees and any late charges or penalries have been
paid in full and written notice of the lifting of said suspension has been provided to the licensee
by the Office of License, Inspections and Environmental Protection.
This Resolution and the action taken above aze based upon the facts contained in the
December 2, 2003 Notice of Violation letter to the licensee. The licensee did not contest the
facts of the violation.

Avenue, in the City of Saint Paul

Dear Ms. Elliott:
The Office of License Inspections and Environmental Protection (LIEP) has
recommended adverse action against the above-referenced license. The basis for the
recommendation is as follows:
On October 20, 2003, you were sent a letter from the Office of
LIEP indicating that your Massage Center license had expired
and that license fees were due. Xou were given until November
17, 2003 to submit the fees but as of today's date, nothing has
been received, nor have you indicated that you no longer wish
to do business in Saint Paul.
The Office of LIEP will recommend the suspension of your ]icense as a massage center until fees
have been paid.
At this time you have three options on how to proceed:
If you do not dispute the above facts and wish to avoid further adverse action, you must
submit the required fees and late charges to the Office of LIEP, Room 300 Lowry
Professional Building, 350 Saint Peter Street, Saint Paul, Minnesota 55102, no later than
Friday, December 12, 2003. The information should be directed to the attention of Ms.
Christine Rozek.

2. If you do dispute the above facts, and believe that information has been submitted, you
can either resubmit the information, or request a hearing, which will be scheduled before
an Administrative Law Judge. At that hearing both you and the City will be able to
appear and gresent witnesses, evidence, and cross-examine the other s wimesses. The St.
Paul City Council will ultimately decide the case.
3. If you wish to cease doing business as a massage practitioner in Saint Paul you can send a
letter to the Office of LIEP indicating your intention to cancel your license.
If you have not contacted me by December 12, 2003, I wiil assume that you aze not contesting the
facts and will schedule this matter for a hearing before the City Council for a consent agenda, at
which time your license will be suspended until such time as the fees are paid and the
information is submitted.
