Dear Tom: - Attached is amendment to the master policy providing £or the reduction in rates, the increase in the maximum amount of coverage under the plan and a change in the renewal dat e of the policy. You will note that these changes are effective January 1 rather than March 1, as indica�ed in my letter to you dated 4ctober 11, 1967. The reason for this change in date is that other changes in �he City Health and Welfare Plan are , effective January 1� and that a general resolicitation of City employees comrnenced in January. This change to the date of January 1 will make the reduced rates and increased amounts of coverages available to the employees before �iarch 1. In addition, the renewal date of January 1 for this policy will coincide with the xenewaZ date of the city-pay p1an, Policy 2881-G. Assuming that this change in dates will be approved by the City Counci.l, we are going ahead ith the calculation of a ten-month dividend for the periocl March 1, 1967 to�� 1968. Hereafter, the dividends under this policy will be computed for a full twelve-month period beginning each January 1. PAUL, MINNESOTA. This amendment is effective as of January 1, 1968. � . 1. On page 1 of the policy, the section entitled "Term Period and Renewal" -<� � has been amended to read as follows: �r � . "Term Period and Renewal. This policy is issued for a term of one year beginning on the date hereof at twelve o'clock noon central standard time. Unless discontinued by the City, it shall be renewed auto- matically on the first day of October, 1946, and in each subsequent year up to and including the year 1954. This policy shall be renewed °•� automatically on the first day of March, 1955, and in each subsequent year up to and including the year 1967. This policy shall thereafter " be renewed automatically on the first day of,January of each year be- ginning in 1968. Such renewals shall be made without medical examina- tion, provided not less than one hundred empl.oyees nor less than seventy-five per cent of all eligible employees are insured hereunder at the beginning of each policy year.'� r�;• __ �1 2. On page 2 of the policy, under the section entitled "Schedule of Group '�` Life Insurance", the second paragraph is amended to read as fcllov�s: '�Every employee who insures hereunder shall be insured in the basic amount of $1,000. An additional $1,000 of gxoup life insurance or any multiple thereof up to $14,000 will be granted any eligible employee who makes application and furnishes evidence of insurability satisfactory to the Company. Evidence of insurability shall be fur- nished at the employee's expense unless he makes application for the additional insurance at the time he applies for the basic $1,000 of insurance.
