i WHEREAS, adverse action was initiated against the Provisional Taacicab Vehicle License

WHEREAS, the licensee did not respond to the Notice of Revocation to contest the
allegations; and
WHEREAS, the Notice of Revocation stated that if the licensee failed to challenge the
denial by October 18, 2004, the Provisional Tasicab Vehicle License would be revoked by the
Office of L�P now, therefore, be it

1615 - 4`" Street South in Minneapolis.
License ID #20040001293
Deaz Mr. Hassan:
The Office of License, Inspections and Environxnental Protection (LTEP) has
recommended adverse action against the provisional taxicab vehicle license held by Mohamed A.
Hassan for the City of Saint. The basis for the recommendation is as follows:
You received a notice on September 16, 2004 from the OFfice
of LIEP indicating that you had failed three sections of the
Taxicab Driver Training Course, and that you were required
repeat the entire course the next time it is given and retake
the entire test.
You were also informed that you needed to pay the $55.00 course
fee by Thursday, September 30, 2004. As of today, you have not
rescheduled the training course nor have you made the $55.00 course
fee payment.
The recommendation is for revocation of your provisional license until such time as you
make the $55.00 course fee payment and repeat the Taxicab Driver Training Course.
At this time you have three options on how to proceed:
If you do not dispute the above facts but wish to proceed with your license
application, you may submit payment and reschedule the Taxicab Driver Training
Course with the Office of License, Inspections and Environmental Protection at
350 Saint Peter Street, Suite 300, Saint Paul, Minnesota, 55102 no later than
Monday, October 18, 2004. Information should be directed to the attention of
Christine Rozek.

2. If you do not dispute the above facts please send me a letter with a statement to
that effect. The matter will then be scheduled for a hearing before the St. Paul
City Council to determine what penalty, if any, to impose. You will have an
opportunity to appeaz and speak on your own behalf, or to have someone appeaz
there for you. The recommendation from the licensing office is for the immediate
suspension of your license until a certificate of insurance has been submitted and
approved by the licensing office.
3. On the other hand, if you wish to dispute the above facts, I will schedule an
evidentiary hearing before an Administrative Law Judge (ALJ). If you wish to
have such a hearing, please send me a letter stating that you are contesting the
facts. You will then be sent a"Notice of Hearing," so you will lmow when and
where to appear, and what the basis for the hearing will be.
In either case, please let me know in writing no later than Monday, October 18, 2004
how you would like to proceed. If I have not heard from you by that date, I will assume
that you are nof contesfing the facts of the violation. The matter will then be scheduled for
the St. Paul City Council and placed on the Consent Agenda during which
no discussion is allowed and the recommended penalty will be imposed.
