Resolved, that upon proper execution and delivery of a release in full to the City of Saint Paul,
the proper City officials are hereby authorized and directed to pay from the Tort Liability
Fund (09080-0511), to Sia and Mai Thao and their attorneys, Chesmut and Cambronne, the
sum of $10,500.00, in full settlement of any and all claims, for damages sustained on
October 17`�, 2001 as a result of an accident at or near the intersection of Avon and Thomas in
Saint Paul, Minnesota.

On October 77, 2001, a police,squad driven by Officer Timothy Bohn came out of an alley near the intersection of Avon and
Thomas and broadsided a vehicle driven by Sia Thao. A settlemeni has been reached with Ms. Thao and her attorneys, Chestnut
and Cambronne in the amount of 510,500. '

In sole consideratian of the payment of ten thousand five hundred dotlars and no
cents (b 10,500.00), to us paid in hand, we do hereby release and forever discharge the
City of Saint Paul, the Saint Paul Police Department, Timothy Bohn, their representatives,
successors, assigns and ali other persons, firms and corporations from any liability, claims,
actions, causes of action, and demands of any kind, known or unknown, existing or to
arise in the future, resulting from or refated to any damage, loss or injury sustained by me
arising from an incident which took place on or about 77�' day of October, 2001 at or
near the intersection of Avon and Thomas, Saint Paul, MN.
We understand that any injuries sustained may be permanent and progressive, and
that recovery may be uncertain. We rety only on our own judgment in making this release

agree to release the City and the other parties named above from any and all claims
including, but not limited to, those claims for medical bills, medical expenses, therapy or
chiropractic treatment, lost wages, legal expenses or costs, personal expenses, and loss
of consortium. We understand that by signing below we release the above named parties
from any and all claims from any provider of any service or treatment in connection with
this claim and that we are responsible for any unpaid bili or invoice connected with injury
or damage from this event.
Tfie payment of this money is not to be construed as an admission of liability. It
represents only the compromise of a doubtful and disputed ciaim.
This release contains the entire agreement between the parties hereto, and the
terms of this release are contractual and no# a mere recital.
