RESOLVED, that pursuant to the Award of the Minnesota
Industrial Commission, dated February 8, 1971, John T. Verhulst,
employee of the Department of Public Safety, Bureau of Police,
be paid from the Workmen' s Compensation Account of the General
Fund, the sum of �2,'756.25, representing 61 .25 weeks of compensation
at the compensation rate of �45.00 per week, subject to the conditions
set forth below; such sum being payable by reason of the permanent
partial disability of said John T. Verhulst who was injured on
June 1, 1967, while employed by the Department oY Public Saf ety,
Bureau of Police; be it
FURTHER RESOLVED, that out of the aforesaid compensation,
25% or �689.00 be deducted and paid directly to Gerald B. Forrette,
attorney for said John T. Verhulst in full satisfaction of legal
services rendered and the remaining balance of the aforesaid
compensation, �2,06'7.25, be paid directly to said John T. Verhulst
in one lump sum without interest deduction and, further, that
Dr. Meyer Z. Goldner be paid the sum of �110.00 for a medical
examination performed on said John T. Verhulst and, further, that
the sum of �55. 13, representing two percent (2�) of the permanent
partial disability of said John T. Verhulst, be paid to the
Workmen' s Compensation Commission on behalf of the Special Compensation
Fund.
