RESOLVED, that Delores Ponshock, widow of Burton G. Ponshock,
employee of the Department of Public Works, who died from an
occupational disease on October 27, 1971, be paid from the
Workmen' s Compensation Account of the General Fund, the sum of
�80.00 per week, payable every four weeks, commencing as of
October 28, 1971 , and continuing during dependency, until the
sum of �30,000.00 shall have been paid, together with reimbursement
in the sum of �744.00 for funeral expenses incurred and paym�nt
to Dr. Markle Karlen for services rendered in the amount of �150.00;
that �2, 128.00 in partial settlement, to and including May 2, 1972,
is now payable; and be it
FURTHER RESOLVED, that out of the aforesaid compensation,
the employer, City of Saint Paul, shall deduct and pay directly
to Gerald B. Forrette, attorney for Delores Ponshock, the sum
of �532.00, for one-fourth attorney' s fees to and including May 2,
1972, and 25% of the periodic compensation recovered, as and for
attorney' s fees in this matter.
