RES4LVED, that pursuant to the Award of the Minnesota
Industrial Commi�ssion, dated February 7, 1972, John Ehrman,
employee of the Department of Public Safety, Bureau of Police,
be paid from the Workmen' s Compensation Account of the General
Fund, the total sum of �3,500.00, representing 14 weeks of
compensation at the compensation rate of �45.00 per week for a
10� permanent partial disability of the left foot sustained November
21, 1964, and an additional 45. 5 weeks of compensation at the
compensation rate of �63.00 per week for 13� permanent partial
disability of the back sustained November 2g, 1969, subject
to the conditions set forth below; such sum being payable by reason
of the above permanent partial disabilities of said John Ehrman,
who was injured on said dates while employed by the Department
of Public Safety, Bureau of Police; be it
�.� - �""^fFUR.THER RESOLVED, that out of the aforesaid compensation,
25f or �875.00 be deducted and paid directly to Gerald B. Forrette,
attorney for John Ehrman, in full satisfaction of legal services
rende�e�k--and t�x� rema'rni�g bala�ce a� the aforesaid compensatioM,
�2,625.00, be paid directly to said Johr�. Ehrman in one lump sum
without interest deduction and, �urther, that Dr. Meyer Z. Goldner
be paid the sum of �107.50 for a medical examination performed
on said John Ehrman and, further, that the sum of �70.00, representing
two percent (2�) of the permanent partial disabilities of John
Ehrman, be paid to the Workmen' s Compensation Commission on behalf
of the Special Compensation Fund.
