t�IHEREr1S, the parties have agreed to extend the contract performance period to revise
� the amount obligated anci available for payment, and to clarify the reports and manuals
to be delivered under the contract.

This rep�rt shall include a description of the public and private
organizations involved in HI�DS delivery systems in the St. Paul area
covering: (1) m aterial on stiuctur•es and responsibilities; (?) an
analysis of the changes in jurisdictional responsibilities generated
.. by state ].egislative action subsequent to project a��rard: and (3) a.:.
discussion of existing stai;e legislation and its impact on the design
of an inte�rated municipal information system.
III. A data access control plan consisti�g of a manual of policies and procedures
for controlling access to the data in the HRDS data base.
N. A report covering the survey of procedures to be used for the description
of large s'cale i nformation systems.
V. Report .covering '�onceptualization of the St. Paul HRDS.
This report shall cover the problems encountered in attempting to con—
ceptualize a human resources development subsystem and their solutions.
V'I. Report on dai;a management system requirments.
This report shall cover the specificati.ons for a computer operated data
management softc•rare system required to handle the data processing system
conceptualized for HRDS.

VII. " The Contractor shall submit the IiftDS Project Final �teport taking into
consideration that the projec�; has been a research and developrnent
� effort. That there have been both failures an� successes involved with
the project and that� regardle�s of its outcome, it is vital that all
aspects of the project be ar�alyzed. Tn �;his way subsequent projects
with similar objectives can benefit and have a chance to duplieate
successes but not failures. To this end the project f.inal report
(journal) shall be develope� covering the S'ollo���ing areas:
A. Chronicle the events of the project from beginni_ng to end to
serve as a basis for the evaluati.on of these events and the
azialysis of the relations involved �rrhich r„ay categorize them
. as successes, failures or something in beti�een.
B.. The evaluatior� of the method and direction taken at various points
in the project that affected the project as a irhole.
�� An evaluation of the consortium concept and its helpin�; or hindering
of the project effort. This evaluation shall be directed at the
actual members of the consortium and their adequacy to fulfi]_1
, project goals.
D. _ An analysis of the role and effect of USAC on the project. This
shall include the evaluation of the objectives and procedures .
imposed and utilized by USAC as vie�•red by the project. This
analysis by necessity shall be biased because the only way it can
- be valid is to be vieV�ed from the local project.
�;� Through the analysis of the process and events of the project,
- develop the way'through the use of eYpez•ienced hind—sight to help
. the next prejec� ��o avoid the pitfalls of the present one.
The report shall be completed and submitted within l� months from the
date of this amendmen�;.
VIII. Data Catalogue
The effort concerned with the �catalogue and its two files — the Data
Catalogue and the Decision Catalogue — shall involve the follot�ring
basic tasks:
A. Validation of the information to be collected to describe the data
and its flow.
B. Complete development of decision attributes and develop the Decision
Catalogue.

C. Develop a complete a set of reporl;J/�1.Jplays tha{; support the
anal�rS'1S of data flow, decisions and combination of the tiao.
It is very important to assure that the catalogue can trace an
. item of dat,a throu�;h a system reflecting all that happens to it.
D. Develop a test�demonstration of the catalogue using a relatively
large a.mount of data.
E. Set up t.he compl.ete de]_iverable package — documentatzon, procedures
and pro�;-rarn.
IX. Implement the de���onstration project. (F.S.I�I,S.)
The follo�aing tasl;s shall be coarpleted to provide (1) a completely
demonstrat•ional system; (2) a deliverable and transferable pacicage;
(3) documentation of the package; and (l�) initiation of operation of
the system.
A. Comn_lete all legal arranoements so tha�; all three :invol;red a�encies
, can exchange data.
B. Get clients for each agency on system.
C. Finalize all procedures in association vaith agency and city
persoru�el so that the agencies and city can continue operation
of system and share its operatir�g costs.
D. Formally demonstrate operating system.
E. Use operating system as a test for the use of personal identifier
and geocoding descriptors. Tnis shall also involve �;he application of
� any graphics developed by the University of Minnesota.
F. Validate the specified management and statistical reports and
develop additional reports if the need arises.
G. Complete all operating and training documentation, procedures�as
well as programs in the form necessary for delivery and transfer. �
X. Graphics study and reports
The following reports and products snall be produced:
A. Review existing software mapping programs and furnish a report.
B. Provide an analysis of mapping applications to health and furnish
a report.
C. Provide �brtran mapping prograrn for the demonstration, with program
specifications.
D. Provide a three dimensional bar chart program, with program
speci£ications (Fortran) . -

XI. �Consulting and evaluation a�sistance
The primary professional staff of the St. Paul project com-nittee shall
be available for consul�;atian and evalua�;ion assistance throu�hout the
period of contract perforirance up to a total of two man months of effort.
Such assistan;;e shall involve di_scussir�g pr�oject history, its current
sta�;us, and its possible ex�r�nsion or transfer to another jurisdi.ction.
XII. University of_ i•iin.nesota fin�l projPCt evaluation report
The University of T�;innesota, operating as an independent member of �;he
consortitun, shall prepare a report entitled: Univers�ty of I�ir�nesota
�raluation, St. Pau1 USAC t.rRJ.; Project. _
1^he report shall be representative of the Universityts participation
in the St. Pau1 US�iC Human Resources Development Subsystem contract
effort. Emphasis shall be placed on reason.s for the University�s
successes and failures in participating in this Project. In order to
do this, the history of the project shall be detailed, including the
�rorkings of US�:C and �he local cor.sortium participants. Finally, �;he
team sha11 a�,tempt to outline the elements of an ideal s�.tuation c��hich
might have produced a more successful HRDS Subsystem p-roject.
This report shall be in the arca of overall project evaluation. It
is expected to be produced independently of the Final Project report
(Task VIT) .
