RESQLVED, that the City Contractor - General Installation license (License ID No.
20000004363) held by Castlewall, Inc. d/b/a Castlewall, Inc., is hereby suspended immediately
for failure to maintain a bond and insurance coverage as required for licensure. Said suspension
shall remain in effect until such time as the licensee submits (1) an appropriate License, Permit &
Performance Bond and (2) provides proof of general liability insurance coverage, and written
notice of the lifting of said suspension has been provided to the licensee by the Office of License,
Inspections and Environmental Protection.
This Resolution and the action taken above are based upon the facts contained in the

Dear Sir/Madam:
The Office of License, Inspections and Environmental Protection (LIEP) has
recommended adverse action against the City Contractor license held by Castlewall, Inc.. The
basis for the recommendation is as follows:
On November 29, 2001 the Office of LIEP sent you notice that
the bond which is a requirement of your business licease in
Saint Paul was expired, as was your insurance covera;e. You
were asked to submit proof of replacement coverage by
December 10, 2001. You have failed to respond to that letter
and LTEP has received no information about a current bond or
insurance policy.
If you do not dispute the above facts please send me a letter with a statement to that
effect. The matter will then be scheduled for a hearing before the St. Paul City Council to
determine what penalty, if any, to impose. You will have an opportunity to appear and speak on
your own behalf, or to have someone appear there for you. The recommendation from the
licensing office is for the immediate suspension of your license until an appropriate bond and
proof of insurance is provided and approved by the licensing office.
On the other hand, if you wish to dispute the above facts, I will schedule an evidentiary
hearing before an Administrative Law Judge (ALJ). If you wish to have such a hearing, please
send me a letter stating that you are contesting the facts. You will then be sent a"Notice of
Hearing," so you will kno�v when and where to appear, and what the basis for the hearing will be.

In either case, please let me know in writing no later than Thursday, January 24,
2002, how you would like to proceed. If I have not heard from you by that date, I will
assume that you are not contesting the facts of the violation. The matter will then be
scheduled for the St. Paul City Councii and placed on the Consent Agenda during which no
discussion is allowed and the recommended penalty will be imposed.

Saint Paul, our office has determined that your bond and insurance has expired. In order to
remove the hold that has been placed on your license, the following requirements must be met:
• Submit a License, Permit & Performance bond in the amount of $2,500.00 for the period of
August 29, 2001 through July 7, 2002. The bond must have no lapse in coverage. (Note:
The bond expiration date must coincide with the license expiration date of July 7, 2002 or be
filed as continuous.)
• Submit proof of general liability coverage for the period of July 7, 2001 through 7uly 7, 2002.
The insurance certificate must have no lapse in coverage. (Note: The insurance policy

basis for the recommendation is as follows:
On November 29, 2001 the Office of LIEP sent you notice that
the bond which is a requirement of your business licease in
Saint Paul was expired, as was your insurance covera;e. You
were asked to submit proof of replacement coverage by
December 10, 2001. You have failed to respond to that letter
and LTEP has received no information about a current bond or
insurance policy.
If you do not dispute the above facts please send me a letter with a statement to that
effect. The matter will then be scheduled for a hearing before the St. Paul City Council to
determine what penalty, if any, to impose. You will have an opportunity to appear and speak on
your own behalf, or to have someone appear there for you. The recommendation from the
licensing office is for the immediate suspension of your license until an appropriate bond and
proof of insurance is provided and approved by the licensing office.
On the other hand, if you wish to dispute the above facts, I will schedule an evidentiary
hearing before an Administrative Law Judge (ALJ). If you wish to have such a hearing, please
send me a letter stating that you are contesting the facts. You will then be sent a"Notice of
Hearing," so you will kno�v when and where to appear, and what the basis for the hearing will be.

Saint Paul, our office has determined that your bond and insurance has expired. In order to
remove the hold that has been placed on your license, the following requirements must be met:
• Submit a License, Permit & Performance bond in the amount of $2,500.00 for the period of
August 29, 2001 through July 7, 2002. The bond must have no lapse in coverage. (Note:
