Your license application for a Massage Practitioner license can not be processed until our office receives the fallowing: Submit proof of general and professional liability insurance coverage with an effective date of June, 2001. (Proof should not have a lapse in coverage. If there is a lapse, a letter should be submitted stating that you were not performing massage therapy during the lapsed period.) The City of Saint Paul must be named as an additional insured, with a 30-day norice of cancellation.
