On June 21, 2001 you were sent a Notice of Violation alleging that you did not have
proof of a Performance Bond which is required as part of the General Contractor license in Saint
Paul. You responded by letter dated June 29, 2001 that this matter would be taken care of at the
beginning of July. As of today's date, neither my office nor the Office of License, Inspections
and Environmental Protection has received any information regarding a current Performance
Bond for Quick Set Panels Corporation.
If I have not heard from you by August 7, 2Q01 we will proceed with the original intent to
suspend the license held by Quick Set Panels Corporation in Saint Paul. Please feel free to
contact me if you have any questions.

The Office of License Inspections and Environmental Protection (LIEP) has
recommended adverse action against the City Contractor - General Installation License held by
Quick Set Panels Corporation in Saint Paul. The basis for the adverse action is as follows:
You were advised by letter dated May 21, 2001 that your
Performance Bond had been canceled effective April 27, 2001.
The performance bond is a requirement of the license
referenced above. You were given until May 29, 2001 to
provide proof of a valid Performance Bond but as of today's
date no such proof has been received by LIEP.
If you do not dispute the above facts please send me a letter with a statement to that
effect. The matter will then be scheduled for a hearing before the St. Paul City Council to
determine what penalty, if any, to impose. You will have an opportunity to appear and speak on
your own behalf, or to have someone appear there for you.
On the other hand, if you wish to dispute the above facts, I �vill schedule an evidentiary
hearing before an Administrative Law Judge (ALJ). If you wish to have such a hearin2, please
send me a leiter stating that you are contesting the facts. You wi11 then be sent a"Notice of
Hearing," so you will know �vhen and where to appeaz, and what the basis for the hearing will be_

In either case, please let me know in writing no later than Monday, July 2, 2001,
how you would like to proceed. If I have not heard from you by that date, I will assume

On June 21, 2001 you were sent a Notice of Violation alleging that you did not have
proof of a Performance Bond which is required as part of the General Contractor license in Saint
Paul. You responded by letter dated June 29, 2001 that this matter would be taken care of at the
beginning of July. As of today's date, neither my office nor the Office of License, Inspections
and Environmental Protection has received any information regarding a current Performance
Bond for Quick Set Panels Corporation.
If I have not heard from you by August 7, 2Q01 we will proceed with the original intent to
suspend the license held by Quick Set Panels Corporation in Saint Paul. Please feel free to

The Office of License Inspections and Environmental Protection (LIEP) has
recommended adverse action against the City Contractor - General Installation License held by
Quick Set Panels Corporation in Saint Paul. The basis for the adverse action is as follows:
You were advised by letter dated May 21, 2001 that your
Performance Bond had been canceled effective April 27, 2001.
The performance bond is a requirement of the license
referenced above. You were given until May 29, 2001 to
provide proof of a valid Performance Bond but as of today's
date no such proof has been received by LIEP.
If you do not dispute the above facts please send me a letter with a statement to that
effect. The matter will then be scheduled for a hearing before the St. Paul City Council to
determine what penalty, if any, to impose. You will have an opportunity to appear and speak on
your own behalf, or to have someone appear there for you.
On the other hand, if you wish to dispute the above facts, I �vill schedule an evidentiary
hearing before an Administrative Law Judge (ALJ). If you wish to have such a hearin2, please
send me a leiter stating that you are contesting the facts. You wi11 then be sent a"Notice of
Hearing," so you will know �vhen and where to appeaz, and what the basis for the hearing will be_

In either case, please let me know in writing no later than Monday, July 2, 2001,

The Office of License Inspections and Environmental Protection (LIEP) has
recommended adverse action against the City Contractor - General Installation License held by
Quick Set Panels Corporation in Saint Paul. The basis for the adverse action is as follows:
You were advised by letter dated May 21, 2001 that your
Performance Bond had been canceled effective April 27, 2001.
The performance bond is a requirement of the license
referenced above. You were given until May 29, 2001 to
provide proof of a valid Performance Bond but as of today's
date no such proof has been received by LIEP.
If you do not dispute the above facts please send me a letter with a statement to that
effect. The matter will then be scheduled for a hearing before the St. Paul City Council to
determine what penalty, if any, to impose. You will have an opportunity to appear and speak on
your own behalf, or to have someone appear there for you.
On the other hand, if you wish to dispute the above facts, I �vill schedule an evidentiary
hearing before an Administrative Law Judge (ALJ). If you wish to have such a hearin2, please
send me a leiter stating that you are contesting the facts. You wi11 then be sent a"Notice of
Hearing," so you will know �vhen and where to appeaz, and what the basis for the hearing will be_
