Washington was issued a provisional operator license by the Office of 10 LIEP and was notified of the taacicab driver training course to begin on February 5, 2001, but 11 12 13 14 15 failed to attended and successfully complete the course; now, therefore, be it RESOLVED, that the provisional operator license (license ID # 20000000234) issued to W.C. Washington is hereby revoked for failure to enroll in and successfully complete the required taxicab driver training course.
