On January 5, 1999, a City of Saint Paul squad was damaged when an uninsured motorist made an unprotected left turn in front
of the squad car. The passenger in the squad, officer Chad Dagenais was injured. A bodily injury settlement under the �
uninsured/underinsured ponion of the City's insurence was reached for policy limits of $25,000.00.

For, and in consideration of, the sum of twerrty five thousand dollars and no cents
(525,000), receipt ofi which is hereby acknowtedged,l refease and forever discfiarga the
City of Saint Paul, its principals, agents and representatives, from any and all rights,
claims, demands and damages of any kind, known or unknown, existing or arising in the
future, resulting firom or retating to any injuries or damages suffered, compensab{e under
their priman/ uninsured / underinsured motorist covera9e, within the meaning of Minnesota
Statute Section 65B.49, Subd. 6[e] which is available or may be available to me in
connection with or arising out of certain personal injuries atlegedly received by Chad

I understand that this is a full and comptete settlement of aft claims of every nature
and kind whatsoever havi�g to do with uninsured / underinsured motorist benefits provided
by the City of Saint Paul as a result of the accident cited above, but is not an admission of
liability by the City of Saint Paul that such benefits are owed.
It is my intent to release the City of Saint Paul, its principals, agents and
representatives, from any and all claims which I may have for any damages suffered by me
which are uncompensated because my total damages exceed the residual bodily liability
limit of any other driver or owner of a motor vehicle against whom I am legally entitled to
recover damages. I further agree that I will not make any additional or further claims
against the City of Saint Paul, its principals, agents or representatives, fo� any uninsured /
wderinsured motorist tienefits in connection, with this accident of January 5, 1999.
This release fias no effect �pon, nor is it intended to preclude, Chad Dagenais' claim
for excess uninsured / underinsured motorist bene�ts from other companies or entities.
