WHEREAS, the City of Saint Paul and Independent School District #625 !
provides life and accidental death and dismemberment ins.urance to equal annual
salary less $5, 000 for certain employees, whose bargaining unit negotiated such
insurance for their entire unit as are listed in the specifications, and
WHEREAS, the City received bids from various life insurance companies to
provide such insurance on Bix #A8412-B which was opened on November 28, 1 g79, and
WHEREAS, the tabulation of such bids from the Purchasing Agent indicate
that the low bidder to comply with all the terms of the specifications is Fidelity Mutual I
Life Insurance Company, represented by the First Agency of Hastings with a monthly I
rate of $ . 32 life and $ . 04 accidental death and dismemberment for a total of $ . 36/1000
insurance, now, therefore, be it �
RESOLVED, that said Fidelity Mutual Life Insurance Company be directed '
to issue a life insurance contract to the City of Saint Paul to become effective the First
day of January, 1980, in compliance with all fhe provisions of the specifications for
providing such insurance upon which said bids were received, and be it
FURTHER RESOLVED, that the proper City Officials are directed to pay the ''�
premiums for this insurance as they become due, and be it ',
FINALLY RESOLVED, that the City Clerk be directed to send copies of this
resolution to Fidelity Mutal Life Insurance Company and the First Agency of Hastings.
