Mr. President and Honorable Members:
0'Connor Enterprises, Inc. currently holds an On Sale
Liquor and miscellaneous licenses issued to them at 369 Cedar
Street. The stockholders are Jacqueline Kay 0'Connor and
Michael Timathy 0'Connor.
The above persons and David R. Maddox request that these
licenses be 'transferred to Entertainment Enterprises, Inc. with
David R. Maddox as the sole officer and the sole stockholder at
the same address.
He will operate this business personally and it will
continue to be known as the Blue Chip.

RE: Blue Chip License
Pursuant to Council File 268798, Ordinanee 16391, I am declaring my intention
to refrain from' voting on the proposed liquor license transfer of 0'Connor
Enterprises, Inc. , 369 Cedar, to Entertainment Enterprises, Inc., at the �
same address.
The facts of the proposed transfer"'are clear due to a conflict of interest
on my behalf as I shall be the holder of the license and the only stock-
holder of the corporation.
