The City shall pay to the Grantee its �and the County's contribution in instailments as follows: (a) 25% of the total contribution of both the City and County on or before January 1 , 1979; (b) 25� of the total contribution of both the City and County on or before April 1 , 1979; (c) 25� of the total contribution of both the City and County on or before Ju1y 1 , 1979; (d) 25% of �he total contribution of both the City and County on or before October 1 , 1979� 6. The City, acting as the transmittal agent of the contribution of both the City and County, as set forth in Paragraph 5, shall not be liable to pay to the Grantee tfie County's contribution if not received by it from the County pursuant to Paragraph 4. 7. The Graq�ee shall on April Z0, July 20, October 20, 1979, and ' January 20, 198Q submit to the City an itemized statement of all Grantee's expenditures, revenue and reports of services on a form or forms to be pro- vided by the City. The City shall make such reports availabae to the County upon receipt. Further, the Grantee shall preserve all documentation used by it in complying with the statements and reports required by this paragraph and shall be available to the City and County for its review and audit as desired. 8, The Grantee shall , with approval of the City, develop and enforce a fee schedule for patient charges. 9. The City shall act as the liaison to the Grantee for such day-ta-day administration as may be needed and shall be responsible for: (a) Reviewing and evaluating the program and fiscal activities of : the Grantee and disseminating reports thereof; - 2 - (b) Providing technical assistance to the Grantee in developing and implementing the fee schedule referenced in Paragraph 8; (c) Coordinating activities common to the Division of Public Health of the City and Grantee; � (d) Rendering to the Grantee such other assistance as within the City's resources; (e) Providing for utilization by Grantee by the City's purchasing faci�lities if appropr�ate and applicable. 10. The Grantee shall insure that: (a) Services provided to eligible individuals are furnished without regard to race, color, creed, sex, age, marital status or family size; ____.. __ ._ _ (b)----Services are provided with respect for individual privacy and dignity; and (c) Services are provided without coercion and shall not be denied on the basi�s of refusal to participate. 7n research projects or other activities of the Grantee, or on the basis of ability to pay. 11. The Grantee shall obtain and keep in force at its own expense during . the term of this Agreemen�, professional liability insurance in the amount of Three Hundred Thousand Dollars ($300,000). A certificate evidencing such - ,' insurance as being in force during 1979, and co-insuring the City_ and County, shall be furnished to the City Attorney for review and approva� on or before January l, 1979. 12. The Grantee's organization and activities shall conform to the format described in the attached Exhibit B. 13. The Grantee shall submit to the City and County a line item budget and a list of its services ta be provided� during the term of this Agreement on January 1 , 1979. - 3 - 14. The Grantee shall have available relevant background and qualification summaries of both regularly employed and volunteer staff to the City and County. 15. The Grantee agrees that deviations of ten percent (10%) or more, upwards or downwards, from its 1979 agency budget, and/or additions or deletions from its 1979 operational plan as approved by the City of Saint Paul and Ramsey County will be transmitted to the City of Saint Paul and Ramsey County in a timely fash3on and in a manner to be determined by the City's Clinic Caordinator. The Grantee shall insure that: (a� Services provided to eligible individuals are furnished without regard to race, color, creed, sex, age, marital status or family size; (b) Services are provided witM respect for individual privacy and dignity; and (c) Services are provided without coercion and shall not be denied on the basi5� of refusal to participate� in research projects or other activities of the Grantee, or on the basis of ability to pay. 11. The Grantee shall obtain and keep in force at its own expense during the term of this Agreement, professional liability insurance in the amount of Three Hwndred Thousand Dollars ($300,000). A certificate evidencing such insurance as being in force during i979, and co-insuring the City and County, shall be furnished to the City Attorney for review and approval on or before January 1, 1979. . 12. The Grantee's organization and activities shall conform to the forma� described in the attached Exhibit B. 13. The Grantee shall submit to the City and County a line item budget and a list of its services to be provided during the term of this Agreement on January 1, 1979. - 3 - 14. The Grantee shall have available relevant background and qualification summaries of both regularly employed and volunteer staff to the City and County. 15. The Grantee agrees that deviations of ten percent (1Q�) or more, upwards or downwards, from its 1979 agency budget, and/or addit,ions or deletions from its 1979 operational pian as approved by the City of Saint Paul and Ramsey County will be transmitted to the City of Saint Paul and Ramsey Caunty in a timely fashion and in a manner to be determined by the City's Clinic Coordinator. The City, acting as the transmittal agent of the contribution of both the City and County, as set forth in Paragraph 5, shall not be liable to pay to the Grantee the County's contribution if not received by it from the County pursuant to Paragraph 4. 7. The Grantee shall on April 20, July 20, October 20, 1979, and January 20, 1980 submit to the City an itemized statement of all Grantee's expenditures, revenue and reports of services on a form or� forms to be pro- � vided by the City. The City shall make such reports available to��the County upon receipta Further, the Grantee shall preserve a11 documentation used by it in complying with the statements and reports required by this 'paragraph and shall be available to the City and County for its review and audit as desired. 8. The Grantee shall , with approval of the City, develop and enfvrce a fee schedule for patien�t charges. 9.
