RESOLVED, that the proper City officials are hereby authorized and directed
to execute an agreement with the County of Ramsey whereby the County wi I I
provide the City with the services of Shirley Fenrick Bowens, R.N., Nurse
Clinician, in the Model Cities Health Clinic's geriatric health program for a
period of one year commencing August 1, 1979; the County to be paid a sum
6ased on actual salary, P.E.R.A. , F.I.C.A, and health insurance costs as
listed in Attachment A to the agreement by the City for said services.

CONTRACT
BETWEEN THE CITY OF SAINT PAUL,
AND
SAINT PAUL-RAMSEY MEDICAL CENTER
This contract is made and entered into this day of
August, 1979, by and between the City of Saint Paul, Division of Public Health,
Model Cities Health Program, hereinafter referred to as the City, and the Saint Paul-
Ramsey Medical Center, hereinofter referred to as the Center.
WHEREAS, the City is the grantee of Title III Older Americans Act funds for
the provision of certain health services to the elderly in the Summit-University area,
and the provisions of this program require the services of a nurse clinician position
that is not an existing title under the City civil service rules, and
WHEREAS, the Center has such a qualified nurse clinician available, and it
is deemed to be in the best interest of the City to contract for this position,
NOW THEREFORE, BE IT AGREED, that the City will contract with the Center
for the services of Shirley Fer�rick Bowens, R.N., Nurse Clinician, to perform the
duties and responsibilities of this position as outlined in the approved Title III Older
Americans Act grant application.
Payment for services rendered under this contract shall be based on actual
salary, P.E.R.A., F.I.C.A, and health insurance costs as listed in Attachment A.
Payment shall be made on a monthly basis upon receipt of invoice from the Center.

This contract shall be effective from August 1, 1979 through June 30, 1980.
The contract may be renewed for a period of one year contingenfi on the continued
availobility of funding from Title III of the Older Americans Act or other sources.
