RESOLVED, that the proper City officials are hereby authorized and directed
to execute an agreement with the Independent School District #625 whereby
.
the City will provide the School District with school health services in the
non-public schools from September 1 , 1979 to June 13, 1980; the City to be
paid by the School District a sum not to exceed the funding allocation for
health services to non-public schools in Saint Paul as determined by the Minne-
sota Department of Education.

AGREEMENT
THIS AGREENiENT, made and entered into this �i�C day of
�� 19 �� , by and between Independent School District
#625 , hereinafter referred to as the "School District", and the City
of Saint Paul , a municipal corporation of the State of Minnesota,
hereinafter referred to as the "Contractor",
��VHEREAS, the School District has the responsibility for pro-
viding health services to non-public schools under the provisions of
bi.S. 123. 931-123.937 (1978) ; and is permitted to contract for the
provision of such services ; and
WHEREAS, the Contractor has traditionally provided the services
to non-public schools in Saint Paul and has the staff and facilities
to continue to provide these services ; and it is considered in the
best interest of the parties hereto, and in the public welfare to con-
trac�: for these services ,
NOW, THEREFORE, the Contractor agrees to provide healfih serv-
ices to all eligible non-public school students in Saint Paul in con-

formity with M.S. 123.931, Section 10 , Subdivision 11, and M.S. 123. 935,
Subdivision 2 . These services include but are not limited to: � indi-
vicival health assessment , vision and hearing screening, scoliosis
screening, first aid, throat culturing, communicable disease asse�sment
and reporting, dental assessment, fluoride brushings and referral for
treatment and follow-up.
The Gontractor shall provide the School District with a program
and budget for the 1979-80 schoal year wherein the budget does nat
exceed the State funding allocation for health services to non-public
sch.ouls in Saint Paul.
The Contractor shall further provide the School District with
c{uarterly progress and expenditure reports , a year-end program summary
and expendi�ture report on or before July 10 , 1980 , and other reports
as rec{uiYed by the Minnesota Department of Education ar the School
�iistxict.
The ContractUr agrees to the following adclitional provisions :
1 . Camplete sections D, E, F, G and H on Form No. ED-01434-01
that is provided by the State Department of Education �nd
submit to Independent School Dis�rict #625 .

2 . Submit detailed budget performance reports on a quarterly
basis to Independent School District #625 all direct costs
incurred relative to the program; and to submit analysis
of costs incurred for administration (indirect costsj
along with quarterly report.
3. Submit final expenditure report on or before July 10 , 1980 ,
to Independent School District #625 , using State reporting
forms .
Independent School District #625 agrees to the following
provisions :
1. Reimburse the contractor for program costs incurred on a
quarterly basis upon receigt of detailed performance
report described above (indirect costs limited to 40 of
the total expenditures for the program) . Payments shall
be made to Contractor after Independent School District
#625 has xeceived payments from the State Department of
Education.
FURTHER, the Contractor shall be reimbursed for eligible ex-
penditures provided that such expenditures do not exceed the �otal
allocations authorized to the School District.
The Contraator will insure that eligible health services are
available to all participating non-public schools and students and
that no servics is denied on the basis of race, color, sex, age,
language , religion, nationality or economic status .
THIS AGREEMENT covers the period from September 1 , 1979 to
June 13, 1980.
IN WITNESS WHEREOF, the School District and the Contractor have
executed this Agreement as of the day and year written belaw.
