The Commission agrees to p^ovi3e �naterna7, and infant care services • to certain residents of the City Of Saint Paul in accordance with the appli- cable laws and regulations and professional standards of health delivery as provided by the state and federal governments. 2. The Commission agrees to provide at least 1,071 clinic visifis in the high school clin ic program at the clinic rate (approved by Minnasota Medical Assistance) of $32.68. These visits will be for the purposes of prenatal care, family planning services, Early Periodic Screening and/or disease prevention and control. 3. For specified services rendered, the Commission fee shall not , exceed $35,000.00 for the period of this Agreement. No other compensation shall be paid. . -2- 4. The Commission agrees to maintain and furnish the City quarterly progress reports regarding the responsibilities noted in #1 above. The Commission shall coordinate the performance under this agreement with the City's Division of Public Health, Department of Community Services. 5. The City shall act as the liaison to the Commission for such day-to-da;r ac�inistration as may be needed and shall be responsible for: 3. Coordinating activities common to the Division of Public Health of the City and Commission and the MIC/HS Program. 6. The Commission agrees that: A. Services provided to eligible individuals are furnished without regard to race, color, creed, sex, age, marital status or family size; B. Services are provided with respect for indi- vidual privacy and dignity; and . . -3- C. Services are provided without coercion and shal�, no-t be denied on the basis of refusal to participate in research projects or other activities of MIC or on the basis o� ability to pay. 7. The Commission agrees to save harm].ess, ide�ify and defend the City up to the limits of $100,000.00 per person or $300,000.00 per . occurence from any and all liability claims or demands arising out of the performance of�the Commission or its personnel or its duties -and services under this Agreement. � � 8. The MIC/HS Program shall subm'it to the City comprehensive docum- entation of the justification for the MIC clinic rate and a comprehensive detailing of the servcies �to be provided "during the term of this agreement on January 1, 1979. ' 9. The MIC/HS Program shall have available relevant background and qualification summaries of both regularly employed and volunteer staff to the City., �0. The MIC/HS Program agrees that additions or� deletions from its 1979 operational plan relating solely to this contract's performance as approved by the City Of Saint Paul will, be transmitted to the City Of Saint Paul in a timely fashion and in a reasonable manner for the City's approval. The MIC/HS Program declares Dr. vaura Edwards to be the person responsible for compliance with the terms of thQS Agreement, and its Dr. Laura Edwards as the person responsible for its medical services. 12. T'ne City declares its Director of the Department of Community Services or such �si�ee as no�ed in writing by him as the person respon- sible foT co��liazce with this Agreement. 13. The City does not assume �y responsibility or liability for any acts or omissions of the officers, employees, or agents of other than its own employees. 15. Either party may fierminate this Agreement upon giving thirty (30) days' written notice. Charges which have accrued for services rendered y shall survive any terminafiion of this Agreement. 16. This Agreement shall be governed by the laws of the State of Minnesota, and constitute the entire Agreement for �urnishing MIC/HS services between the Commission and the City. 17. Nothing in this Agreement shall be construed as limiting the right of independent operation of either the Commission or the City or the affiliation or contract with any other institution or agency while the Agreement is in effect. - : ��e���� . PAUL, MN FOR HEALTH EAtTC�►TION SERVICES THIS AGREEMENT, made and entered into this first day of January 1979, by and between the County of Ramsey, a political subdivision of the State of Minnesota, hereinafter refarred to as Ramsey County, and the City of St. Paul, a municipal corporation of the State of Minnesota, hereinafter referred to as the Contractor. WHEREAS, the Contractor has the staff and facilities for the techni- cal assistance and preparation of health education materials and Ramsey County presently has no such staff available to it; and it is considezed in the best interest of the Qarties hereto,. and in the public welfare to contract for certain services. NOW THEREFORE, the Contractor agrees to provide at its Public Health Center, one full time equivalent person for technical assistance and pre- paration of health educatioa and public relations materials as outlined in Attachment A. Ramsey County agrees to reimburse the Contractar in a total sum not to exceed�� q 4 for such services for a period of twelve (12) months commencing on January I, 1979. The Contractor agrees to submit to the Ramsey County Co�¢nunity Health Services, 906 American Center Building, 150 East Kellogg Boulevard, St. Paul� a quarterly report of services provided. Upon receipt of such report, paysaent will be made by Ramsey County within thirty (30) days. � This Agreement may be cancelled by either party at any time, with or without cause, upon giving thirty (30) days notice, in writing delivered by mail or in person. Salary, Health Educator II - with M.A. . . . . . $17,418.00 23 pay periods at C step, 3 pay periods at next step 3,139.94 fringe 52�,557.94 250.00 paper, postage, newstetter $20,807.94 TOTAL The Health Education Section wiil be accountable to Ramsey County �Cors�nunity Heatth Services for roughly the following distribution of time and effort. 1) Fifty percent time spent in public rslations and publicity efforts. Attached you will find �a set of goals and objectives that have been devetoped by Mark Reidell in conjunction with Ramsey County Nursing Service PR corflmittee. 2) Forty percent time spent in health education for suburban Ramsey County residents. This responsibility will be assumed with areas of concentratiort in early periodic screening, immunizations, RCNS health education committee participation and promotion, consultation and resource sharing in community and school health education programs throughout Ramsey County. More detailed goals and objectives in these areas are being developed and will accompany the fourth quarter health education reporz. 3) Ten percent time spent in developing, organizing and/or presenting staff education programs. RCNS will be invited to participate, along with Division of Public Health personnei , in a very thorough needs assessment of staff training needs, followed by planning, implementation and evaluation phases. The needs assessment is expected to take approximately from January through June. Planning will take place over the summer, with programming to being in the fall .
