WHEREAS, many studies have been conducted in Ramsey County regarding
the feasibility of establishing a county-wide public health system in order to
decrease duplication of effort, maximize available resources, strengthen programs,
and improve continuity of services; and
WHEREAS, the combined City of Saint Paul and County of Ramsey Community
Health Services Advisory Committee at its meeting of February 6, 1980 recommended
merger of the Ramsey County and City of Saint Paul public health departments as
per the Community Health Services Ad Hoc Task Force recommendation on City-
County health merger, which report is attached hereto for reference; and
WHEREAS, the Saint Paul City Council believes that the creation of a
oounty-wide public health system could serve the interest of all Saint Paul residents,
and result in the provision of more adequate public health services; Now Therefore

RESOLVED, that the Saint Paul City Council hereby approves the exploration
of the proposed merger of the Saint Paul Division of Public Health and the Ramsey
County Public Health Department; and Be It Further

RESOLVED, that prior to any actual merger proceeding, the following
areas of concern be resolved to the satisfaction of the Saint Paul City Council:
1 . Philosophy of services and delivery.
2. Level of services
3. Administrative structure of inerged organization.
4. Legal provisions for transfer.
5. Protection of transferred employees' rights and benefits.
6. Special projects ordinance.
7. City representation on the Community Health Services Advisory Committee.
8. Transfer of bui Iding and equipment.
9. Transfer of contracts and grants.
10. City ordinances.
11 . Food inspection and licensing.
12. Status of Occupational Safety and Health. ,
AND, BE IT FURTHER RESOLVED, that the Mayor and the Director, Department
of Community Services, City of Saint Paul, in cooperation with the Ramsey County
Executive Director, shall develop a proposal for the merger of the programs of the
City and County health departments as recommended by the Community Health Services
Advisory Committee, and submit such proposal to the Saint Paul City Council and
Ramsey County Board prior to implementation.

SUBJECT: City-County Health Services Merger
The City and County public health units' merger may take place within the next 18 months.
Prior to any merger there are various areas of concern that should be resolved to ensure the
effective operation of such a new agency and the adequate delivery of services in St. Paul.
Listed below are some of these concerns.
1 . Philosphy of Services - What will be the County's philosophy on health services?
Will it tend toward a delivery of traditional public health services? Will there be
a generalized or specialized nursing program, or a combination? What will be the
relationship of the merged health unit to the five neighborhood clinics? Will the
County provide the funding of the clinics, or will it still be shared by the City?
Will the County assume responsibility for the Model Cities Health Clinic?
2. Level of Services - The City should seek assurances as to the level of services that
wi II be maintained in the city proper. Without such assurances it is possible that
the City health unit would be transferred and have the responsibility of serving the
entire county in many program areas without additiona) staff or resources. This
would obviously reduce service delivery in the city.
3. Administrative Structure - A previous proposal called for the director of the Ramsey
County Public Health Department to be director of the merged organization, with
the director of the St. Paul agency being an associate director. This proposal does
not recognize the history, size or scope of the two organizations. With about 170
or more City employees transferring to a County structure, it is highly important
from a morale standpoint that their past city leadership be recognized in the new
organization.
4. Provisions of Transfer - Transfer of the City unit to the County should be accomplished
under the provisions of M.S. Chapter 145, the Community Health Services Act of
1976. This act provides for the protection of existing employees' right and benefits,
which is a highly important item by itself.

5. Protection of Employee Rights - In any merger the transferred employees should be
assured of equivalent positions, pay grades and seniority in the new organization.
Also, there should be no loss in the type and level of health and welfare, annual
leave and sick leave benefits.
6, Special Projects Ordinance - At the present time the City has an ordinance that
permit expenditures in federal or state grant projects based on the "notice of grant
award" and prior to actual deposit of federal or state funds. This feature allows
our agency to be very responsive to emerging public health needs of the community
without waiting to have cash in hand before the program can start. The County
apparently does not have such a mechanism in place, and this could adversely affect
the responsiveness of the new agency. The rapid response time of the City agency
in meeting needs generated by the recent i,nflux of Southeast Asian refugees is a
oase in point.
7. CHS Advisory Committee - The City should provide a mechanism to ensure itself of
a proper representation on the CHS Advisory Committee after the merger. Without
such a mechanism the City could lack adequate representation on the committee.
8. Buildinct and Equipment - An equitable transfer of the Public Health Center and
program furnishings and equipment must be worked out. The Public Health Center
was recently appraised at a market value of $3,800,000. The furnishings and
equipment have an undepreciated value of $531,896, based on the inveqtory dated
December 14, 1979,
9. Contracts and Agreements - The City health unit has numerous granh, contracts and
agreements that would have to be transferred to the County. These transfers could
be very complex, requiring a legal advisor.
� 10. City Ordinances - The City has a body of ordinances pertaining to food establish-
ments and environmental nuisances. The County would need similar legislation to
provide for public health enforcement.
11 . Food Inspection and Licensina - Food inspection in the City is inextricably tied to
licensing of establishments. If this activity is transferred, will certain licensing
revenues also be transferred to cover the cost of inspection?
12. Occupational Safety and Health - The question here is whether this should become
a joint City-County function. To date there are no clear signals as to its ultimate
status.
These areas of concern are brought to your attention in the hope that they will be satis-
factori ly addressed and resolved prior to any merger. Such action on the part of both City
and County officials would lend itself to a smooth and orderly transition of public health
programs and services in St. Paul and Ramsey County.
