INITIATING ACTION: This order was initiated by the Director of Public Works on the basis of a petition with 1 signer representing 100� of the frontage and inspection of the walk. EXISTING CONDITIONS: This walk is poured concrete with tripping hazards , asphalt patches , disintegrated� cracked concrete panels and scaled, tipped and settled panels. Remarks: Only a short stretch of integ�al curb planned for near corner. PROJECT: RECONSTRUCT SIDEWALK North side of EAST THIRD STREET from Flandrau Street to Kennard Street. ItJITIATING ACTIOtJ: This order was initiated by the Director of Public Works on the basis of a petition with 3 signers representing 80� of the frontage and inspection of the walk. EXISTING CONDITIUPJS: This walk is poured concrete with tree heaves � high joints , tripping hazards , asphalt patches , cracked and scaled concrete panels. PROJECT: RECONSTRUCT SIDEWALK South side of CAST THIRD STREET from Gotzian Street to Johnson Parkway. INITIATI�JG ACTIOPJ: This order was initiated by the Director of Public Works on the basis of a petition with 5 signers representing 100� of the frontage and inspection of the walk. EXISTING CONDITIONS: This walk is poured concrete with tree heaves , asphalt patches, cracked, scaled and settled panels. CONSTRUCT/ PROJECT: RECOIJSTRUCT SIUEWALK North side of EAST THIRD STREET from 65 feet West of Etna Street to 150 feet West of Etna Street, abutting 1379 and 13II5 East Third Street and South side of EAST THIRD STREET at 1310 and 1318 East Third Street. INITIATING ACTION: This order was initiated by the Director of Public Works on the basis of a petition with 1 signer representing 50�G of the frontage and inspection of the walk. EXISTING CONDITIOF�S : This walk is an asphalt trail on the north side and 5 poured concrete panels on the south side which are cracked and settled. This is a M.S.A. street so it is not assessed even though �here are some 80' of new construction. It will be financed y�P I A funds. 2. F.�����V ` PROJECT: RECOIJSTRUCT SI,DEWALY. East side of BARCLAY STREET between York Avenue and Case Avenue at 930 Barclay Street. INI'fIATING ACTIO�J: This order was initiated by the Director of Public Works on the basis of a petition w� th 1 signer representing 100$ .of the frontage and inspection of the walk. EXISTING CONDITIONS: This walk is old tile with broken tile, high joints, water pockets , offgrade and with asphalt patches. PROJECT: RECONSTRUCT SIOEWALK East side of HAZELWOOD AVENUE between McLean Avenue and Pacific Street at 31 Hazelwood Avenue. INITIATING ACTION: This order was initiated by the Director of Public Works on the basis o a petition with 1 signer representing 100$ of the frontage and inspection of the walk. EXISTING CONDITIUNS: This walk is poured concrete with tree heaves, tripping hazards and settled panels. PROJECT: RECONSTRUCT SIDEWALK Both sides of WILSON AVENUE from Cypress Street to East 0 feet at 1005 and 100 Wilson Avenue only. INITIATING ACTION: This order was initiated by the Oirector of Public Works on the basis of a petition with 1 signer representing 60� of the frontage and inspection of the walk. EXISTING COWDITIONS: This walk is poured concrete with tree heaves , asphalt patches , tipped panels and a disintegrated driveway crossing at 1004 Wilson. PROJECT: RECONSTRUCT SIDEWALK Both sides of MARGARET STREET from Earl Street to Cypress Street. INITIATING ACTION: This order was initiated by the Director of Public Works on the basis of a petition with a signers representing 50� of the frontage and inspection of the walk. EXISTING CONDITIDNS: This walk is 40' of broken tile and poured concrete with tree heaves, high joints, tripping hazards , longitudinaly cracked concrete panels and scaled, tipped panels. PROJECT: RECONSTRUCT SIDEWALK South side of WEST HOYT AVENUE from 30 feet West of No�th Victoria Street to 90 feet East of Fisk Street (vac) INITIATING ACTION: This order was initiated by the Director of Public Works on the basis of a petition with 3 signers representi�g 80�6 of the frontage and inspection of the walk. EXISTIN� CONDITIOtJS: This walk is poured concrete with asphalt patches , cracked concrete panels and scaled and settled panels.
