WHIl�EAS, the City of Saint Paul provides for. a comprehensive Health and Welfare
Insurance Program for city employees and employees of Independent School District �625, as
provided for under Council File No. 267621, as amended, and
WHF�tEAS, the present �5,000 basic life Insurance coverage is provided. by Republic
National Life Insurance Company of Dallas, Texas, and
WHF,REAS, pursuant to the provision af the existin� contract, the City negotiated
the follo��ring monthly premiums for a term of 12 months, October l, 1981 to September 30, 1982,
and be it
RESOLVID, that Minnesota Mutual Life Insurance Company will bill the City at the
rate of $2.07 each month for each eligible insured and remit �2.00 to the Republic National
Life Insur3.nce Company and retain $ .05 for their administration and remit � .02 to the
Haxry R. Ochs Agency for their services, an� be it
FURTHIl� RESOLVID, that said Republic National Life Insurance Company be directed to
continue a life insura,nce contract vrith the City of Saint Pau1, and be it
FURTHE'R RESOLVID, that the proper City Officials are directed to pay the premiums
for this insurance as they become due, a.nd be it
FIlVALLY RESOLV�, that the City Clerk be directed to send copies of this resolution
to the Republic National Life Insura,nce; Anchor Agency, Incorporated; Minnesota Mutual Life
Insurance Company and Ochs A.�ency, Incorporated.
