These services include but are not limited to: individual health assessment, vision and hearing screening, scoliosis screening, first aid and emergency care, communicable disease assessment and reporting, im►nunization status revie�a and reporL-in�s, �ental assess- ment, and referral for treatment�and follo�a-up. The Contractor shall provide the School Distr.ict with a program and budget for the 1980-81 school year wherein the budget does not exceed the State funding allocation for health s�rvices to non-public schools in Saint Paul. The Contractor shall further provide the School District with quaiterly progress and expenditure reports, a year-end program summary and e�penditure report on or before July 10, 1981, and other reports as required by the Minnesota Department of Education or the School llistricto � - . � � ����s _ 2 _ The Contractor agrees to the Lollowing additional provisions: _ 1. Complete sections D, E, F, G and H on Form No, ED01434-01 that is pro- vided by the State Department of Education and submit to Independent School District ��625. 2. Submit detailed budget performance reports on a quarterly basis to In- depender:.t School District ��625 all direct costs incurred relative to the prograrn; and to submit analysis of costs incurred for administra- " tion (indirect costs) along with quarterly report. 3, Submit final expenditure report on or before July 10, 1981, tv Indepen- dent School District ��625, using State reporting forms. Independent School District ��625 agrees to the following provisions: 1. Reimburse the contractor for program costs incurred on a quarterly basis upon receipt of detailed performance report described above (indirect costs limited to 4% of the total expenditures for the program). pay_ ments shall be made to Contractor after Independent School District ��625 has received payments from the State Department of Education, FURTI�F, the Contractor shall be reimbursed for eligible expenditures pro- vided that such expenditures do not exceed the total allocations authorized to the School Districto - The Contractor will insure that eligible health services are available to all participating non-public schools and students and that no service is denied on ttie basis o� race, color, sex, age, language, religion, nationality or eco- � I nomic status.
