dated as of the effective date of this Resolution, on file in the office of the
City Clerk, is hereby approved, and the authorized administrative officials
of the City are hereby authorized and directed to execute said "Memorandum

This Memorandu� of Understanding shall apply to the employees holdir_g the
title of Assistant Fire Chief.
l. Salaries applicable ta tt�e title of Assistant Fire Chief shall be as
shown below.

2.. Employees employed in the title of Assistant Fire Chief shall �e covered
by the vacation schedule shown below: ',

3. The employer will continue for the period of this Memorandum to provide �
for employees such health and life insurance benefits as are grovided by '
empZoyer at the tir.ze of the execution of this Memorandum.
The employer will for the period of this Memorandum provide for e�ployees
who retire af ter the time of execution of this biemorandum and until such
employees reach sixty-f ive (65) years of age such health and life insuranice
� benefits as are provided by the employer. In order to be eligible for the
benef its under this early retiree provision, the employee must:

3.1 Be receiving benef its from a Public Employee Retirement Act. ',
3.2 Have severed his relationship with the City of Saint Paul
under one of the early retiree plans. '

4. In the event �f the death of an active employee or an early retiree, the '
dependent of such empZoyee or retiree shall have the option within thirt�
(30) days to continue or begin the current hospitalization and medical ,
benef its, including such improvement as may be made from ti.a:e to time,
which said dependents previously had, or be eligible for the prenium '
applicable to dependents of active employees. It is further understood
that coverage shall cea.se in the event of:
4.1 Subsequent remarriage of the surviving spouse of the deceased
employee ar retiree. ,
4.2 The employ�nent of the surviving spouse where hospitalization
and r.►edical insurance coverage is obtained through a group
program by said employer. It is further understood, however,
that in said event, the surviving spouse shall have the right
to maintain City hospitalization and medical insurance coverage
for the f irst ninety (90) days� of said employment.

5. For each eligible employee covered �y this AgreeLnent G�ho selects
Elue Cross-Elue Shield insurance coverage, the City agrees to contri-
bute the cost of such coverage or $69.70 per month, whichever is less.
In addition, for each employee who selects Blue Cross-Blue Shield
depenc�ent's coverage, the City will contribute the cost of such
dependent's coverage or $161.45 per month, whichever is less.
6. For each eligible employee covered by this Agreera.ent who selects
Group Health insurance coverage, the City agrees to contribute the
cost of such coverage or $42.58 per month, whichever is less. In ,
addition, for each eMployee who selects Group Health dependent`s '
coverage, the City will contribute the cost of such dependent's '
coverage or $51.59 per month, whichever is less.
7. For each eligible employee covered by this Agreement who selects ',
Coordinated Care health insurance coverage, the City agrees to contri- �
bute the cost of such coverage or $56.90 per month, whichever is 7.ess.
In addition, for ea.ch employee wfio selects Coordinated Care dependent's �
coverage, the City will contribute the cost of sucY: dependent's cover-
age or $103.00 per month, whichever is less.

8. For each eligible employee covered by this Agreement who selects �i0- j
Minnesota insurance coverage, the City agrees to contribute the cost ;
of such coverage or $68.96 per month, whichever is less. In addition,
for each eiuployee who selects the HMO-Minnesota dependent's coverage,
the City will contribute the cost of such degendent's caverage or '
$155.91 per month, whichever is less.
9. The City agrees to contribute the cost for $5,000 of Life Insurar.ce ,
Coverage for each employee who is eligible for such coverage or $2.07
per month� whichever amount is less. This coatribution shall be paid ;
to the City's group hea.lth and welfare plan.
i0. In addition to the $5,000 Lif e Insurance Coverage in 9, the City agrees '
to contribute the cost of additiona.l life insurance coverage or $ ,97 ',
per thousand dollars of coverage per month, whichever amount is Iess. �
The .total amount of life insurance coverage provided under this section i
and section 9 for each employee shall be equal to the e�ployee's annual �
salary to the nea.rest full thousand dollars. For the purpose of this
section, the employee's annual salary sha.11 be based on the salary as
of the beginning of a contract period. This contribution shall be paid !
to the City's group hea.lth and welfare plan.
11. Any increase cost for the insurance coverages cited in items 5 thru 10
after September 30, 1982, shall be paid by the Employee.
12. The Residency Resolution effective Auoust 4, 1979, in Council File i�o.
273378 shall apply to all employees covered by this memorandu�. ;

ACTION REQUESTED
We recommend your approval and submission of this Resolution to the City Council.
PURPOSE AND RATIONALE FOR THIS ACTION
This Memorandum changes the existing Memorandum by addi.ng language which grants
the full payment of the October, 1981 insurance premiums. The 1982 wage increase
will be 7% a.nd will not be reduced by the cost of the insurance increase as provided
for in the existing Memorandum.
FINANCIAL IMPACT�
There are two employees affected by this change. The cost amounts to $690. 84 per yea .
